芳香化酶抑制剂相关骨关节痛治疗研究进展

Research Progress in Aromatase Inhibitor-Induced Arthralgia

芳香化酶抑制剂(AIs)是绝经后激素受体阳性乳腺癌标准辅助内分泌治疗。AIs相关骨关节痛(AIA)是AIs治疗最常见的不良反应之一,严重影响患者的生存质量和治疗的依从性。AIA尚无明确治疗指南,运动疗法、补充维生素(维生素D、Omega-3脂肪酸、维生素B12)、针灸治疗为AIA主要非药物治疗措施,疗效值得肯定,已广泛应用于临床。另外,更换内分泌药物也是有效且可行性高的手段。镇痛药、短期激素治疗、中药治疗为AIA主要药物治疗措施,但还需大样本、多中心、证据等级高的临床试验进一步证明其可行性。

Aromatase inhibitors(AIs)are standard of care in the adjuvant endocrine therapy of postmenopausal women with hormone receptor positive breast cancer.AIs-induced arthralgia(AIA)is one of the most common adverse effects which may lead to drug discontinuation and poor quality of life.There is no clear treatment guidelines for AIA,while exercise,vitamin supplements(vitamin D,Omega-3 fatty acids,vitamin B12),acupuncture are the main non-medicine treatments of AIA,which are effective and widely used in clinic.Hormonal therapy switching is also effective with high feasibility.Analgesics,short-term hormonal therapy and traditional Chinese medicine are the main medical treatments for AIA,although largescale,multi-center,high-level clinical trials are still needed to further prove the feasibility.
