口腔修复膜材料在牙种植中引导骨再生的临床效果和安全性 被引量:13

Clinical Efficacy and Safety of Prosthodontic Membrane Materials in Guided Bone Regeneration in Dental Implantation

摘要 目的探讨口腔修复膜材料在牙种植中引导骨再生的临床效果和安全性。方法选取2017年1月至2018年1月在新疆维吾尔自治区人民医院口腔科接受牙种植的患者49例,按照治疗方法不同分为观察组(n=24)和对照组(n=25),观察组采用新型胶原蛋白口腔修复膜材料治疗,对照组采用传统钛膜材料治疗。比较两组患者手术前后的骨缺损情况(骨缺损高度)、修复后植骨厚度与成骨厚度及不良反应的发生情况。结果手术后两组患者的骨缺损高度低于手术前,且观察组的骨缺损高度低于对照组[(2. 1±0. 7) mm比(2. 3±0. 5) mm],两组间、时点间及组间和时点间比较差异均有统计学意义(P <0. 01)。观察组的植骨厚度与成骨厚度显著高于对照组[(2. 51±0. 38) mm比(2. 04±0. 26) mm,(2. 65±0. 52) mm比(2. 23±0. 35) mm](P <0. 01)。两组患者的切口裂开、修复膜暴露、牙周组织红肿发生率比较差异无统计学意义(P> 0. 05)。结论采用新型胶原蛋白口腔修复膜材料行牙种植引导骨再生修复成功率高,能有效促进成骨和植骨的发育成长,改善牙周软组织的健康状况,不良反应发生率低。 Objective To investigate the clinical effect and safety of prosthodontic membrane materials in guided bone regeneration in dental implantation. Methods From Jan. 2017 to Jan. 2018,49 patients who received dental implant in the Department of Stomatology of People’s Hospital of Xinjiang Uygur Autonomous Region were divided into an observation group( n = 24) and a control group( n = 25) according to the different treatment methods. The observation group was treated with new collagen prosthodontic membrane materials,while the control group was treated with traditional titanium membrane materials. The bone defects( height of bone defect),the thickness of osteogenesis,the thickness of bone graft after repair and the occurrence of adverse reactions were compared between the two groups before and after operation. Results The height of bone defect in the two groups after operation was lower than that before operation,and the observation group lower than the control group[( 2. 1 ± 0. 7) mm vs( 2. 3 ± 0. 5) mm],and the differences between the two groups,between time points,and between groups and time points were statistically significant( P < 0. 01). The thickness of bone graft and osteogenesis in the observation group was significantly higher than that in the control group [( 2. 51 ± 0. 38) mm vs( 2. 04 ± 0. 26) mm,( 2. 65 ± 0. 52) mm vs( 2. 23 ± 0. 35) mm]( P < 0. 01). There were no significant differences in incision dehiscence,prosthodontic membrane exposure,and periodontal tissue swelling between the two groups( P > 0. 05). Conclusion The success rate of guided bone regeneration in dental implantation with new collagen membrane material is high,which can effectively promote the development of the bone graft and osteoblastic growth,improve the health of periodontal soft tissue,and has low incidence of adverse reactions.
