卵巢早衰的中西医研究进展

Research Progress of Chinese and Western Medicine on Premature Ovarian Failure

随着生物-心理-社会医学模式的不断深化,从精神心理角度论治卵巢早衰前景广阔。西医治疗卵巢早衰疗效快速、显著,但停药即复发,且有增加乳腺疾病及耗损卵泡的风险。中医治病求本,强调"形神合一",认为心、肝与精神情志密切相关,对于卵巢早衰的调治,要重视身心同治,不拘泥于补肾,多从调畅情志角度出发,注重养心疏肝,创造有助于卵巢功能的内分泌环境。中西医结合治疗卵巢早衰是一种安全有效的临床防治手段,值得临床推广应用,但目前研究缺乏诊断及治疗的统一标准,缺乏大样本的、准确的实验数据,具体作用机制尚未明确。今后需继续开展大规模、多中心的临床观察,针对其具体作用机制进行深入的动物实验,从生物、心理、社会等诸因素入手对卵巢早衰进行全面评估,同时进一步探讨"形神合一"的理论基础,从精神心理角度论述卵巢早衰的发病机制及临床价值。

With the deepening of bio-psycho-social medical model,there is a broad prospect to treat premature ovarian failure from the perspective of psycho and psychology.Western medicine has a rapid and significant effect on premature ovarian failure,but it will recur when the drug is stopped,and it will increase the risk of breast disease and loss of follicles.Traditional Chinese medicine emphasizes the"integration of body and spirit",and believes that the heart,liver and spirit are closely related.For the treatment of premature ovarian failure,attention should be paid to the treatment of both body and mind.We should not confine to invigorating the kidney,but focus on nourishing the heart and soothing the liver,and create an endocrine environment conducive to ovarian function.The combination of Chinese and western medicine in the treatment of premature ovarian failure is a safe and effective means of clinical prevention and treatment,which is worthy of clinical application.However,at present,the research lacks a unified standard of diagnosis and treatment,a large sample of accurate experimental data,and the specific mechanism of action is not clear.In the future,we need to continue to carry out large-scale,multi center clinical observation,conduct in-depth animal experiments for its specific mechanism of action,and start with biological,psychological,social and other factors to conduct a comprehensive assessment of premature ovarian failure,at the same time,further explore the theoretical basis of"integration of body and spirit",and discuss the pathogenesis and clinical value of premature ovarian failure from the perspective of psycho and psychology.
