苓甘五味姜辛汤加减治疗喉源性咳嗽临床疗效观察

Curative effect observation of Linggan Wuwei Jiangxin Decoction in the treatment of laryngeal cough

喉源性咳嗽是干祖望教授首次提出的耳鼻咽喉科疾病,本研究选取40例中医诊断符合脾气虚弱型喉源性咳嗽门诊患者,将其分为治疗组和对照组,治疗组采用苓甘五味姜辛汤加减治疗,药物组成:茯苓12 g,甘草6 g,干姜6 g,细辛3 g,五味子10 g,陈皮10 g,半夏6 g,疗程2周,临床加减为:咽痒明显,咳嗽不能自止,加蝉蜕10 g,防风10 g;咽干口渴,无痰或少痰,去干姜,加麦冬10 g,生白术10 g;咳嗽自汗,加黄芪15 g,白术10 g,咳甚至小便不能自制者,加党参10 g,益智仁10 g,中药采用颗粒剂。西药治疗给予头孢克肟分散片,盐酸左西替利嗪分散片口服。经1个疗程治疗后,观察治疗组与对照组的疗效差异性。根据治疗后结果显示,治疗组总有效率是95%,对照组总有效率是80%。通过治疗前后两组患者症状体征总积分差值的组间比较分析,P<0.05,两组之间的总有效率存在明显的统计学意义,所以得出最终结论为:苓甘五味姜辛汤加减组临床总体效果好于对照组。

Laryngeal cough is otolaryngological disease,which is first proposed by Professor Gan Zuwang.This study selected 40 cases of TCM diagnosis patients with deficiency of Spleen Qi laryngeal cough.They were divided into the treatment group and the control group.The treatment group was treated with Linggan Wuwei Jiangxin Decoction(苓甘五味姜辛汤).Drug composition was as follows:Poria Cocos 12 g;Licorice 6 g;Dried ginger 6 g;Asarum 3 g;Schisandra chinensis 10 g;Poria Cocos 12 g;Licorice 6 g;Dried ginger 6 g;Asarum 3 g;Schisandra chinensis 10 g;Orange peel 10 g;Pinellia tuber 6 g.The course of treatment was two weeks.Addition and subtraction:for severe throat itch and severe cough,Cicada slough 10 g and radix sileris 10 g were added;For dry throat and thirst,no or less phlegm,dried ginger was replaced by ophiopogon root 10g,Rhizoma Atractylodis Macrocephalae 10 g;For cough and spontaneous sweating,Astragalus membranaceus 15 g,Rhizoma Atractylodis Macrocephalae 10 g were added;For uncontrollable urination caused by cough,Codonopsis pilosula 10 g and Alpinia Alpinia 10 g were added.Granules were used in traditional Chinese medicine.Cefixime dispersible tablets and levocetirizine hydrochloride dispersible tablets were given orally.According to the results after treatment,the total effective rate was 95%in the treatment group and 80%in the control group.A comparative analysis of the difference of the total score of symptoms and signs between the two groups before and after treatment was made,P=0.02<0.05,there was significant statistical difference in total effective rate between the two groups.Final conclusion:The clinical effect of Linggan Wuwei Jiangxin Decoction was better than that of the control group.
