消化内镜与四联疗法联合治疗胃溃疡出血的效果研究

Therapeutic Effect of Digestive Endoscopy Combined with Quadruple Therapy on Gastric Ulcer Bleeding

目的探讨消化内镜与四联疗法联合治疗胃溃疡出血的效果。方法便利选择2014年5月—2018年5月该院收治的胃溃疡出血患者,共计116例。116例患者以随机数表法进行分组,分为对照组与研究组各58例。对照组仅采用四联疗法治疗,研究组在对照组的基础上结合消化内镜治疗。评价两组患者的临床疗效、再出血的发生率、治疗前后胃动力学指标(胃动素、胃泌素浓度水平)的变化,以及住院时间。结果两组治疗的总有效率比较中,研究组96.55%高于对照组84.48%,差异有统计学意义(χ^2=4.921,P<0.05)。研究组再出血的发生率为3.45%(2/58),低于对照组17.24%(10/58),差异有统计学意义(χ^2=5.949,P<0.05)。治疗后研究组胃动力学指标胃动素(225.5±20.5)ng/mL与胃泌素(132.5±15.8)ng/mL均低于对照组(288.6±23.6)ng/mL、(155.6±16.0)ng/mL,差异有统计学意义(t=6.565、5.856,P<0.05)。研究组的住院时间为(5.2±1.5)d,低于对照组(9.2±2.3)d,差异有统计学意义(t=4.859,P<0.05)。结论消化内镜与四联疗法联合治疗胃溃疡出血疗效确切,不仅可以快速止血,且调节了胃动力学指标,缩短康复速度。

Objective To investigate the effect of combined digestive endoscopy and quadruple therapy on gastric ulcer bleeding.Methods A total of 116 patients with gastric ulcer bleeding admitted to the hospital from May 2014 to May 2018 were convenienty selected.116 patients were divided into two groups:control group and study group,58 cases by random number table method.The control group was treated only with quadruple therapy,and the study group was combined with digestive endoscopy on the basis of the control group.The clinical efficacy,the incidence of rebleeding,changes in gastric kinetics(motilin,gastrin concentration)before and after treatment,and hospitalization time were evaluated.Results The total effective rate of the two groups was 96.55%in the study group and 84.48% in the control group.The difference was statistically significant(χ^2=4.921,P<0.05).The incidence of rebleeding in the study group was 3.45%(2/58),which was lower than that in the control group 17.24%(10/58).The difference was statistically significant(χ^2=5.949,P<0.05).After treatment,the gastric motility index(225.5±20.5)ng/mL and gastrin(132.5±15.8)ng/mL in the study group were lower than those in the control group(288.6±23.6)ng/mL,(155.6±16.0)ng/mL.The difference was statistically significant(t=6.565,5.856,P<0.05).The hospitalization time of the study group was(5.2±1.5)d,which was lower than that of the control group(9.2±2.3)d.The difference was statistically significant(t=4.859,P<0.05).Conclusion Digestive endoscopy combined with quadruple therapy is effective in treating gastric ulcer bleeding.It can not only stop bleeding quickly,but also regulate gastric motility index and shorten the recovery speed.
