无精症及少精症患者血清生殖激素水平和睾丸生精功能相关性 被引量:3

Correlation between Serum Reproductive Hormone Levels and Testicular Spermatogenesis in Patients with Azoospermia and Oligozoospermia

摘要 目的对无精症及少精症患者血清生殖激素水平和睾丸生精关系分析。方法随机选取在该院2017年8月—2018年8月实施治疗的60例无精症及少精症患者,检测分析患者的血清中T(总睾酮)、FSH(卵泡刺激素)、LH(黄体生成素)、PRL(泌乳素)、E2(雌二醇)各项指标水平,依照睾丸或附睾穿刺病理结果将其分成3组,分别为A组,患者生精功能正常;B组,患者生精功能低下;C组,患者唯支持细胞综合征。结果A组患者的FSH为(5.29±2.41)mIU/mL、LH为(4.19±2.19)mIU/mL、T为(4.53±1.25)ng/mL、PRL为(11.16±0.28)ng/mL以及E2为(35.02±7.88)pg/mL;B组患者的FSH为(8.07±7.83)mIU/mL、LH为(5.21±2.39)mIU/mL、T为(3.28±2.35)ng/mL、PRL为(12.65±0.38)ng/mL以及E2为(38.19±16.15)pg/mL;C组患者的FSH为(16.07±2.61)mIU/mL、LH为(8.03±0.52)mIU/mL、T为(27.94±3.83)ng/mL、PRL为(13.01±0.04)ng/mL以及E2为(31.49±10.52)pg/mL。对比发现,和A、B两组患者对比,C组患者的血清FSH、PRL、LH水平明显偏高,E2水平明显偏低,差异有统计学意义(P<0.05),A组和B组两组患者的生殖激素水平对比,差异无统计学意义(t=1.241、0.245、2.157、1.574、0.587,P>0.05);和A组患者对比,C组患者的血清LH、FSH指标明显偏高,同时T水平明显偏低,差异有统计学意义(t=7.967、8.145、7.691,P<0.05),两组患者的PRL、E2水平差异无统计学意义(t=1.254、0.563,P>0.05),统计发现,患者睾丸生精功能及T水平关系为正相关,和LH、FSH指标水平关系为负相关,和PRL、E2无相关性。结论在无精症及少精症患者的血清生殖激素水平分析过程中,能够发现和睾丸生精功能密切相关,可通过血清生殖激素水平检测评估患者睾丸生精功能,为患者疾病诊断及治疗提供依据。 Objective To analyze the relationship between serum reproductive hormone levels and testicular spermatogenesis in patients with azoospermia and oligozoospermia.Methods Sixty patients with azoospermia and oligozoospermia who were treated in the hospital from August 2017 to August 2018 were randomly selected.The serum of patients with T(total testosterone),FSH(follicle stimulating hormone),and LH(Luteinizing hormone),PRL(prolactin),E2(estradiol)indicators were analyzed,according to the testicular or epididymal pathological results were divided into three groups,respectively,group A,patients with normal spermatogenesis;group B,the patient's spermatogenic function was low;in group C,the patient only supports cell syndrome.Results The FSH of group A was(5.29±2.41),LH was(4.19±2.19),T was(4.53±1.25)ng/mL,PRL was(11.16±0.28)ng/mL,and E2 was(35.02±7.88)pg/mL,FSH was(8.07±7.83)mIU/mL,LH was(5.21±2.39)mIU/mL,T was(3.28±2.35),PRL was(12.65±0.38)ng/mL,and E2 was(38.19±16.15)pg/mL;FSH of group C was(16.07±2.61)mIU/mL,LH was(8.03±0.52)mIU/mL,T was(27.94±3.83)ng/mL,PRL was(13.01±0.04)ng/mL,and E2 was(31.49±10.52)pg/mL.Compared with the A and B groups,the serum FSH,PRL,and LH levels were significantly higher in the C group,and the E2 level was significantly lower.The difference was statistically significant(P<0.05),the difference of reproductive hormone levels between the two groups of patients in group A and B was not significant(t=1.241,0.245,2.157,1.574,0.587,P>0.05);compared with patients in group A of the serum LH and FSH indexes of patients in group C were significantly higher,and the T level was significantly lower.The difference was statistically significant(t=7.967,8.145,7.691,P<0.05).The difference of PRL and E2 levels between the two groups was not obvious,(t=1.254,0.563,P>0.05).Statistics showed that the relationship between spermatogenic function and T level in patients'testes is positively correlated,and negatively correlated with LH and FSH levels,and not correlated with PRL and E2.Conclusion In the analysis of serum reproductive hormone levels in patients with azoospermia and oligozoospermia,it can be found to be closely related to the spermatogenic function of the testis.The testicular spermatogenic function can be assessed by serum reproductive hormone level test for the diagnosis and treatment of patients'diseases.
