家庭参与式个案管理对糖尿病高危人群自我管理能力的影响

Effect of family-involvement case management on self-management skills among high-risk diabetic population

目的探讨家庭参与式个案管理对糖尿病高危人群的健康管理效果。方法112例糖尿病高危人群依据建档顺序分为观察组和对照组,各56例。对照组给予常规健康教育,观察组在对照组基础上予以家庭参与式个案管理。观察2组干预前后糖尿病知识掌握情况、自我管理能力及血糖变化,比较2组对护理管理的满意度。结果干预后,2组糖尿病知识掌握程度及自我管理能力均有提高,且观察组优于对照组(P<0.05)。干预后,2组空腹血糖、餐后2 h血糖水平均降低,且观察组改善优于对照组(P<0.05)。观察组管理满意度96.43%(54/56),高于对照组83.93%(47/56),差异有统计学意义(P<0.05)。结论家庭参与式个案管理应用于糖尿病高危人群健康管理中,可显著提高其糖尿病知识掌握程度及自我管理能力,对稳定血糖水平具有积极作用。

Objective To investigate the effect of family-involvement case management on the self-care management of the high-risk diabetic population.Methods Totally 112 subjects at high risk of developing diabetic mellitus were randomly divided into the observation group and control group,with 56 cases in each group.All participants received routine health education,and those the observation group were given a family-involvement case management.The knowledge about diabetes mellitus and self-management abilities were evaluated before and after intervention.The changes in blood glucose indicators and participants'degree of satisfaction were compared between two groups.Results Participants in the observation group achieved higher scores in assessment of diabetes-related knowledge and self-management abilities compared with those in the control group(P<0.05).The levels of fasting blood-glucose and 2-h postprandial blood glucose were decreased in both observation group and control group after intervention,and were lower in the observation group than those in the control group(P<0.05).The satisfaction rate was 96.43%(54/56)in the observation group and 83.93%(47/56)in the control group,with a significant difference(P<0.05).Conclusion The family-involvement case management is effective to improve the knowledge level and self-management skills among high-risk diabetic population,as well as having a positive effect on blood glucose control.
