祛痰清肺汤辅助治疗小儿支气管肺炎痰热闭肺证67例临床观察

Clinical observation on Qutan Qingfei Tang(祛痰清肺汤) in adjuvant treatment of 67 cases of bronchopneumonia in children with phlegm-heat obstructing lung syndrome

目的观察祛痰清肺汤辅助治疗小儿支气管肺炎痰热闭肺证的临床疗效。方法选取2016年12月至2018年12月在嘉兴市中医医院儿科住院治疗的小儿支气管肺炎痰热闭肺证患儿134例作为研究对象,采用随机数字表法分为治疗组和对照组,各67例。对照组给予常规西药治疗,治疗组在对照组治疗方法的基础上加用祛痰清肺汤治疗,2组均治疗7 d后,比较2组的临床疗效、中医证候积分改善情况、炎性细胞因子水平以及临床症状消失时间。结果对照组总有效率为85.07%,治疗组为95.52%,2组比较,差异有统计学意义(P<0.05)。治疗前,2组中医证候积分比较,差异无统计学意义(P>0.05),具有可比性;治疗后,2组中医证候积分较同组治疗前均明显下降(P<0.05),且治疗组下降更显著(P<0.05)。治疗前,2组γ-干扰素(IFN-γ)、肿瘤坏死因子-α(TNF-α)、白细胞介素-2(IL-2)、白细胞介素-4(IL-4)水平比较,差异均无统计学意义(P>0.05),具有可比性;治疗后2组上述指标均明显改善,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组改善更显著(P<0.05)。治疗组体温恢复正常时间、咳嗽消失时间、肺部啰音消失时间均短于对照组,2组比较,差异有统计学意义(P<0.05)。结论祛痰清肺汤辅助治疗小儿支气管肺炎痰热闭肺证,能快速缓解临床症状,有效调节炎性指标,改善中医证候,提高治疗效果,值得临床推广应用。

Objective To observe the clinical efficacy of Qutan Qingfei Tang(祛痰清肺汤) in adjuvant treatment of bronchopneumonia in children with phlegm-heat obstructing lung syndrome.Methods Totally 134 cases of bronchopneumonia in children with phlegm-heat obstructing lung syndrome hospitalized in pediatrics department of Jiaxing Municipal Hospital of TCM from December 2016 to December 2018 were chosen as research objects and divided into treatment group and control group according to random number table method,with 67 cases in each.The control group was given routine Western medicine treatment, on this basis,the treatment group was added with Qutan Qingfei Tang.After the 2 groups were both treated for 7 days,the clinical efficacy,the improvement of TCM syndrome scores,levels of inflammatory cytokines and disappearance time of clinical symptoms of the 2 groups were compared.Results The total effective rate of the control group was 85.07%,that of the treatment group was 95.52%,and the difference was statistically significant between the 2 groups(P<0.05).Before treatment,the difference was not statistically significant in TCM symptom scores between the 2 groups(P>0.05),and they were comparable.After treatment,the TCM symptom scores of the 2 groups all significantly decreased compared with the same group before treatment(P<0.05), moreover,the treatment group decreased more significantly(P<0.05).Before treatment,the difference was not statistically significant in the levels of interferon-γ(IFN-γ),tumor necrosis factor-α(TNF-α),interleukin-2(IL-2) and interleukin-4(IL-4) between the 2 groups(P>0.05),and they were comparable.After treatment,the above-mentioned indexes of the 2 groups all significantly improved,and the difference was statistically significant compared with the same group before treatment(P<0.05),moreover,the treatment group improved more significantly(P<0.05).The time of body temperature returning to normal,the disappearance time of cough and lung rale in the treatment group were all shorter than those in the control group,and the difference between the 2 groups was statistically significant(P<0.05).Conclusion It can quickly relieve clinical symptoms,effectively regulate the inflammatory indexes,improve TCM syndrome and improve curative effects to use Qutan Qingfei Tang in adjuvant treatment of bronchopneumonia in children with phlegm-heat obstructing lung syndrome,which deserves clinical promotion and application.
