甲状腺乳头状癌Ki-67、BRAF、Galectin-3的表达与淋巴结转移的关系 被引量:1

摘要 目的探讨甲状腺乳头状癌Ki-67、BRAF、Galectin-3的表达与中央区淋巴结转移的关系。方法将168例甲状腺乳头状癌根据中央区淋巴结转移情况分为淋巴结无转移组(n=76)、淋巴结转移1个组(n=40)、淋巴结转移≥2个组(n=52),采用免疫组化法检测Ki-67、BRAF、Galectin-3在各组中的表达,并进行比较。结果Ki-67在淋巴结无转移组中阳性率为51.3%,在淋巴结转移1个组中为72.5%,在淋巴结转移≥2个组中为90.4%。Ki-67阳性率在各组间均有统计学差异(χ^2=4.8485、21.3767、5.0336,P均<0.05),且随着淋巴结转移数的增加,阳性率升高(P<0.05)。BRAF在淋巴结无转移组中阳性率为42.1%,在淋巴结转移1个组中为62.5%,在淋巴结转移≥2个组中为82.7%。BRAF阳性率在各组间均有统计学差异(χ^2=4.3616、20.9635、4.7808,P均<0.05),且随着淋巴结转移数的增加,阳性率升高(P<0.05)。Galectin-3在淋巴结无转移组中阳性率为73.7%,在淋巴结转移1个组中为90.0%,在淋巴结转移≥2个组中为94.2%。Galectin-3阳性率在淋巴结有转移各组中均较淋巴结无转移组中为高(χ^2=4.2516、8.8428,P均<0.05),但在淋巴结转移数不同的两组中无统计学差异(χ^2=0.5757,P>0.05)。结论甲状腺乳头状癌Ki-67、BRAF、Galectin-3的表达与中央区淋巴结转移密切相关,Ki-67、BRAF、Galectin-3高表达者更易出现淋巴结转移。故Ki-67、BRAF、Galectin-3可作为预测甲状腺乳头状癌淋巴结转移的重要肿瘤标志物。 Objective To investigate the relationship between the expression of Ki-67,BRAF and Galectin-3 in papillary thyroid carcinoma and lymph node metastasis in central region.Methods One hundred and sixty-eight cases of thyroid papillary carcinoma were divided into the lymph node metastasis=0 group(n=76),lymph node metastasis=1 group(n=40)and lymph node metastasis≥2 group(n=52)according to status of central lymph node metastasis.Immunohistochemistry was applied to determine and compare the expression of Ki-67,BRAF,and Galectin-3 in three groups.Results The positive rates of Ki-67 were 51.3%in the lymph node metastasis=0 group,72.5%in the lymph node metastasis=1 group,and 90.4%in the lymph node metastasis≥2 group.The positive rates of Ki-67 were statistically different among the groups(χ^2=4.8485,21.3767,5.0336,P<0.05),and the positive rate was increased with the increase of lymph node metastasis(P<0.05).The positive rates of BRAF were 42.1%in the lymph node metastasis=0 group,62.5%in the lymph node metastasis=1 group,and 82.7%in the lymph node metastasis≥2 group.The positive rates of BRAF were statistically different among the groups(χ^2=4.3616,20.9635,4.7808,P<0.05),and the positive rate was increased with the increase of lymph node metastasis(P<0.05).The positive rates of Galectin-3 were 73.7%in the lymph node metastasis=0 group,90.0%in the lymph node metastasis=1 group,and 94.2%in the lymph node metastasis≥2 group.The positive rates of Galectin-3 were higher in the groups with lymph node metastasis than in the lymph node metastasis=0 group(χ^2=4.2516,8.8428,both P<0.05),but there was no statistical difference in the two groups with different number of lymph node metastasis(χ^2=0.5757,P>0.05).Conclusion The expression of Ki-67,BRAF and Galectin-3 in papillary thyroid carcinoma is closely related with lymph node metastasis in central region.Lymph node metastasis is more likely to occur in patients with high expression of Ki-67,BRAF and Galectin-3.Therefore,Ki-67,BRAF,and Galectin-3 can be used as important tumor markers for predicting lymph node metastasis of papillary thyroid carcinoma.
