高频超声对乳腺增生中医辨证分型的临床研究

Clinical Study on TCM Syndrome Differentiation of Breast Hyperplasia by High Frequency Ultrasound

目的分析高频超声对乳腺增生中医辨证分型的临床效果。方法该文研究对象为乳腺增生患者,研究总例数200例,收取时间在2015年2月—2016年2月之间,总例数采取抽签分组方式分为两组,观察组100例(实施高频超声诊断)、对照组100例(实施CT诊断),将两组的中医辨证分型情况、检出率、误诊率和漏诊率、特异性和敏感度进行对比。结果在中医辨证分型中,观察组痰瘀互结型所占比为30.00%;任冲失调型所占比为20.00%;肝郁气滞型所占比为50.00%与对照组相比较,两组之间差异有统计学意义(χ^2=4.700、5.980、13.333;P<0.05)。观察组乳腺增生患者检出率95.00%高于对照组患者75.00%,差异有统计学意义(χ^2=15.686,P<0.05)。观察组乳腺增生患者误诊率2.00%、漏诊率3.00%低于对照组患者(χ^2=5.674,P<0.05)。观察组乳腺增生患者特异性90.00%、敏感度87.00%高于对照组患者(χ^2=10.526、9.441,P<0.05)。结论通过对乳腺增生患者实施高频超声明确中医辨证分型,取得显著效果,能显著提高检出率,值得研究。

Objective To analyze the clinical effect of high frequency ultrasound on TCM syndrome differentiation of breast hyperplasia.Methods The subjects of this study were breast hyperplasia patients.The total number of cases was 200.The collection time was between February,2015 and February,2016.The total number of cases was divided into two groups by lottery group.Observation group 100 cases(implementation of high-frequency ultrasound diagnosis)and 100 cases of control group(implementation of CT diagnosis),the TCM syndrome differentiation,detection rate,misdiagnosis rate and missed diagnosis rate,specificity and sensitivity were compared between the two groups.Results In the classification of TCM syndromes,the proportion of sputum and stasis type in the observation group was 30.00%;the proportion of Ren Chong dysfunction type was 20.00%;the proportion of liver stagnation and Qi stagnation type was 50.00%The difference between the two groups was statistically significant(χ^2=4.700,5.980,13.333;P<0.05).The detection rate of patients with mammary gland hyperplasia was 95.00%in the observation group and 75.00%in the control group.The difference was statistically significant(χ^2=15.686,P<0.05).The misdiagnosis rate of the patients with mammary gland hyperplasia was 2.00%,and the rate of missed diagnosis was 3.00%lower than that of the control group(χ^2=5.674,P<0.05).In the observation group,the specificity of breast hyperplasia patients was 90.00%,and the sensitivity was 87.00%higher than that of the control group(χ^2=10.526,9.441,P<0.05).Conclusion Through the implementation of high-frequency ultrasound in patients with hyperplasia of mammary glands to clear TCM syndrome differentiation,significant results have been achieved,which can significantly improve the detection rate,which is worthy of study.
