针灸推拿联合西药治疗腰椎间盘突出症临床效果观察

Clinical effect of acupuncture and massage combined with Western medicine on the treatment of lumbar disc herniation

目的:探讨针灸推拿联合西药治疗腰椎间盘突出症患者的临床疗效。方法:100例腰椎间盘突出症患者分为两组,各50例。其中对照组给予布洛芬缓释胶囊等治疗;观察组于对照组治疗基础上给予针灸、推拿治疗。于治疗3个月后评价两组临床疗效,评价两组治疗前后主观症状和临床体征评分、疼痛程度、健康状况、腰椎状况。结果:观察组治疗有效率为94.0%明显高于对照组68.0%,差异具有统计学意义(P<0.05);观察组治疗后主观症状和临床体征评分明显低于对照组,且两组治疗后均低于治疗前(P<0.05)。观察组治疗后VAS评分明显低于对照组(P<0.05);两组治疗后VAS评分低于治疗前(P<0.05)。两组患者治疗后JOA评分及各指标评分明显高于治疗前,且观察组高于对照组(P<0.05);两组患者治疗后ODI评分明显高于治疗前,观察组治疗后ODI评分明显高于对照组(P<0.05)。结论:西医治疗基础上联合针灸推拿治疗,可缓解腰椎间盘突出症患者疼痛,改善其临床症状和腰椎功能,提高临床疗效。

Objective:To investigate the clinical effect of acupuncture and massage combined with Western medicine in the treatment of lumbar disc herniation.Methods:A total of 100 patients with lumbar disc herniation were divided into observation group and control group,with 50 patients in each group.The patients in the control group were treated with ibuprofen sustained-release capsules,and those in the observation group were treated with acupuncture and massage in addition to the treatment in the control group.Clinical outcome was evaluated after 3 months of treatment,and the scores of subjective symptoms and clinical signs,degree of pain,health status,and conditions of lumbar vertebra were evaluated before and after treatment.Results:The observation group had a significantly higher response rate than the control group(94.0%vs 68.0%,P<0.05).Both groups had significant reductions in the scores of subjective symptoms and clinical signs after treatment,and the observation group had significantly lower scores than the control group after treatment(P<0.05).Both groups had a significant reduction in Visual Analogue Scale(VAS)score after treatment(P<0.05),and the observation group had a significantly lower VAS score than the control group after treatment(P<0.05).Both groups had significant increases in Japanese Orthopaedic Association score and the scores of each index after treatment,and the observation group had significantly higher scores than the control group(P<0.05).Both groups had a significant increase in Oswestry Disability Index(ODI)score after treatment,and the observation group had a significantly higher ODI score than the control group after treatment(P<0.05).Conclusion:Acupuncture and massage combined with Western medicine can relieve the pain of patients with lumbar disc herniation,improve their clinical symptoms and lumbar function,and thus improve their clinical outcome.
