bcl-10和蛋白表达的关系及其在胃肠MALT淋巴瘤的诊断应用价值

目的探讨bcl-10和蛋白表达及其在胃肠MALT淋巴瘤的应用价值.方法随机选取2014年8月至2018年8月本院胃肠MALT淋巴瘤患者40例,另随机选取同期本院非肿瘤性病变淋巴结28例,将其标本取出来,常规石蜡切片,HE染色.结果40例患者中,表达bcl-10蛋白36例,占总数的90.0%,阳性细胞呈弥漫分布,其中阳性信号只在胞质定位21例,同时表达于胞质胞核15例.Ann Arbor分期Ⅰ期23例患者中,核阳性2例,Ⅱ期12例患者中,核阳性9例,Ⅲ期5例患者中,核阳性4例,核阳性率分别为8.7%、75.0%、80.0%,Ⅰ期患者的核阳性率显著低于Ⅱ期、Ⅲ期患者(P<0.05),表达bcl-10 mRNA 39例,占总数的97.5%,阳性信号主要分布在胞质,少数分布在细胞核.bcl-10蛋白和mRNA表达之间的差异不显著(P>0.05).14例淋巴结正常健康体检人员中,表达bcl-10蛋白8例,占总数的57.1%,阳性信号只在胞质定位.B细胞高表达于淋巴滤泡生发中心,中等强度表达于边缘区,微弱表达于套区细胞.高恶转化型患者的T(11;18)阳性率显著高于低度恶化患者(P<0.05).结论在胃肠MALT淋巴瘤的发生发展过程中,bcl-10高表达可能发挥重要作用.bcl-10和蛋白表达在胃肠MALT淋巴瘤的诊断应用价值高,有效补充了常规病理诊断,进而将有效依据提供给临床治疗,从而有效提升临床疗效,改善患者预后.

Objective To investigate the bcl-10 and protein expressions and their clinical values in gastrointestinal MALT lymphoma.Methods 80 cases of patients with gastrointestinal MALT lymphoma in our hospital from August 2014 to August 2018 were randomly selected,and 28 cases of healthy persons with normal lymph nodes in our hospital during the same period were randomly selected.Their specimens were taken out,routine paraffin sections and HE staining were performed.Results Of the 40 cases of patients,36 cases expressed bcl-10 protein,accounted for 90.0% of the total.Positive cells were diffusely distributed,of which 21 cases were localized in the cytoplasm and 15 cases in the cytoplasm nucleus.Among 23 cases of patients in Arm Arbor stageⅠ,2 cases were nuclear positive,12 cases in stage Ⅱ,9 cases were nuclear positive,and 5 cases in stage Ⅲ,4 cases were nuclear positive.The positive rate of nuclear were 8.7%,75.0% and 80.0% respectively.The positive rate of nuclear in stage Ⅰ was significandy lower than stage Ⅱ and Ⅲ(P<0.05).39 cases of patients expressed bcl-10 gene,accounted for 97.5%of the total.Positive signals were mainly distributed in the cytoplasm and a few in the nucleus.The difference of expression of bcl-10 protein and niRNA was not significant(P>0.05).Among 14 cases of healthy persons with normal lymph nodes,8 cases expressed bcl-10 protein,accounted for 57.1%of the total.Positive signals were localized only in the cytoplasm.B celk were highly expressed in the germinal center of lymphoid follicles,moderately expressed in the marginal zone and weakly expressed in the mande zone.The positive rate of T(11;18)in patients with high malignant transformation was significandy higher than that in patients with low malignant transformation(P<0.05).Conclusion High expression of bcl-10 may play an important role in the development of gastrointestinal MALT lymphoma.The expression of bcl—10 and bcl-10 protein in gastrointestinal MALT lymphoma has high diagnostic value,which effectively complements the routine pathological diagnosis,then provides effective basis for clinical treatment,so as to effectively enhance the clinical efficacy and improve the prognosis of patients.
