维持性血液透析患者压力源与应对方式的调查研究

Investigation of stress sources and coping styles in maintenance hemodialysis patients

目的了解维持性血液透析患者的压力源、应对方式现状及影响因素。方法选取2016年12月—2017年12月山东省4所三级甲等医院的240例维持性血液透析患者,应用血液透析患者压力源量表和简易应对方式问卷进行调查。结果维持性血液透析患者压力总体处于轻度水平,前五位的压力源分别为:疗程长、需长期透析治疗、医疗费用、对未来的担忧、对医生形成依赖。维持性血液透析患者倾向于采取积极应对方式。结论维持性血液透析患者面临病程长、长期透析和高额医疗费用等压力源,这提示医务人员应关注MHD患者的主要压力源,引导男性患者采取积极的应对方式,并积极呼吁政府继续提高MHD人群的报销比率。

Objective:To understand the pressure source,coping style and influencing factors of maintenance hemodialysis patients.Methods:240 patients with maintenance hemodialysis in 4 tertiary hospitals in Shandong Province were investigated by convenience sampling,pressure source scale for hemodialysis patients and simple coping style questionnaire.Results:The stress level of maintenance hemodialysis patients was mild.The top five sources of stress were long course of treatment,long-term dialysis treatment,medical expenses,worries about the future,dependence on doctors and hypofunction.Maintenance hemodialysis patients tend to adopt positive coping styles.Conclusion:Maintenance hemodialysis patients are faced with pressure sources such as long course,long-term dialysis and high medical costs,which suggests that medical staff should pay attention to the main pressure sources of MHD patients,guide male patients to adopt positive coping styles,and actively appeal to the government to continue to improve the reimbursement rate of these groups.
