2%碳酸氢钠口腔护理对呼吸窘迫综合征机械通气新生儿VAP发生率的影响

Effect of 2%Sodium Bicarbonate Oral Care on the Incidence of VAP in Neonates with Mechanical Ventilation in Respiratory Distress Syndrome

目的探讨2%碳酸氢钠口腔护理对呼吸窘迫综合征(Respiratory distress syndrome,RDS)机械通气新生儿呼吸机相关性肺炎(Ventilator associated pneumonia,VAP)发生率的影响。方法选取我院RDS患儿79例(2017年2月至2019年7月),均行机械通气,2017年2月至2018年3月39例为对照组,给予生理盐水口腔护理,2018年4月至2019年7月40例为观察组,给予2%碳酸氢钠口腔护理。比较两组VAP、口腔感染发生率、机械通气时间、住院费用、住院时间。结果观察组VAP、口腔感染发生率2.50%(1/40)、5.00%(2/40)低于对照组20.51%(8/39)、25.64%(10/39,P<0.05);观察组机械通气时间、住院时间短于对照组,住院费用少于对照组(P<0.05)。结论2%碳酸氢钠口腔护理应用于RDS机械通气新生儿,可有效降低VAP、口腔感染发生率,促进患儿康复,减少住院费用。

Objective To investigate the effect of 2%sodium bicarbonate oral care on respiratory distress syndrome(RDS)incidence of ventilator-associated pneumonia(Ventilator associated pneumonia,VAP)in neonates with mechanical ventilation.Methods Selected 79 children with rds in our hospital(february 2017 to july 2019),all performed mechanical ventilation,39 cases from february 2017 to march 2018 as the control group,given saline oral care,40 cases from april 2018 to july 2019 as the observation group and 2%sodium bicarbonate oral care.compared the two groups of vap,incidence of oral infection,mechanical ventilation time,hospitalization expenses,hospitalization time.Results The incidence of oral infection in the observation group was 2.50%(1/40),5.00%(2/40)was lower than that in control group 20.51%(8/39)and 25.64%(10/39,P<0.05).The duration of mechanical ventilation in the observation group was shorter than that of the control group,and the hospitalization cost was less than that in the control group(P<0.05).Conclusion The application of 2%sodium bicarbonate oral care in RDS mechanically ventilated neonates can effectively reduce the incidence of VAP,oral infection,promote the recovery of children and reduce the cost of hospitalization.
