乳腺癌PICC置管患者导管维护依从性及导管相关并发症发生的影响因素

Influencing Factors of Catheter Maintenance Compliance and Catheter-related Complications in Breast Cancer Patients Undergoing PICC Catheterization

目的研究乳腺癌经外周静脉穿刺中心静脉置管(PICC)期间患者导管维护依从性及导管相关并发症发生的影响因素及相关性。方法选取2017年3月~2019年5月我院乳腺癌PICC置管患者549例,统计导管维护依从率和导管相关并发症发生率。采用Logistic方法分析导管维护依从性及导管相关并发症发生的影响因素,并分析导管维护依从性与导管并发症的相关性。结果导管维护低水平依从性162例,占29.51%,导管相关并发症发生64例,占11.66%。单因素分析结果显示,年龄、文化水平、经济状况、家庭支持度、医疗支付方式、心理状态是导管维护依从性的影响单因素(P<0.05)。年龄、文化水平、糖尿病、家庭支持度、贴膜类型、瓣膜类型、固定方式、导管维护依从性是导管相关并发症发生影响因素(P<0.05)。多因素Logistic分析结果显示,文化水平、经济状况、家庭支持度、心理状态是影响乳腺癌PICC置管期间导管维护依从性的风险因素(P<0.05),糖尿病、贴膜类型、瓣膜类型、固定方式、导管维护依从性是导管相关并发症发生的风险因素(P<0.05)。导管维护依从性与导管移位、局部感染、堵管和接触性皮炎发生呈负相关(P<0.05)。结论乳腺癌PICC置管期间患者导管维护依从性及导管相关并发症发生影响因素多样,其中导管维护依从性是导管相关并发症发生的风险因素之一,且二者存在一定相关性。

Objective To study the influencing factors and correlation of catheter maintenance compliance and catheter-related complications in breast cancer patients undergoing PICC catheterization.Methods 549 patients with PICC of breast cancer from March 2017 to May 2019 were selected.The compliance rate of catheter maintenance and the incidence of catheter-related complications were counted.Logistic method was used to analyze the compliance of catheter maintenance and the influencing factors of catheter-related complications,and the correlation between the compliance of catheter maintenance and catheter-related complications was analyzed.Results There were 162 patients with low-level catheter compliance,accounting for 29.51%,and 64 patients with catheter-related complications,accounting for 11.66%.Age,literacy level,economic status,family support,medical payment methods,and mental state were the single factors of catheter maintenance compliance(P<0.05);age,literacy level,diabetes,family support,film type,valve type,Fixation and catheter maintenance compliance were single factors affecting catheter-related complications(P<0.05).Multivariate logistic analysis confirmed that education level,economic status,family support and psychological status were risk factors affecting catheter maintenance compliance during PICC catheterization for breast cancer(P<0.05).Diabetes mellitus,patch type,valve type,fixation method and catheter maintenance compliance were risk factors for catheter-related complications(P<0.05).The compliance of catheter maintenance was negatively correlated with catheter displacement,local infection,occlusion and contact dermatitis(P<0.05).Conclusion There are various factors influencing the compliance of breast cancer patients with PICC catheterization and catheter-related complications.Catheter maintenance compliance is one of the risk factors for catheter-related complications,and there are certain correlations between them.
