柴术消痞汤治疗胃痞病临床研究

Clinical Research of Chaizhu Xiaopi Decoction on Stomach Bloating

目的研究柴术消痞汤治疗胃痞病临床疗效,为中医药防治胃痞病提供新的思路和方法。方法选取胃痞病患者120例,随机分为治疗组和对照组各60例,治疗组采用"柴术消痞汤"治疗,对照组采用多潘立酮每次10 mg,每日3次治疗,两组用药均为4周,在治疗结束后第1个月进行随访。观察两组治疗前后临床症状变化,统计主症、次症积分,研究"柴术消痞汤"临床疗效。结果柴术消痞汤治疗胃痞病的有效率为93.33%,显著高于对照组有效率(72.88%)(P<0.01)。在中医证候疗效和胃脘胀满、胃脘疼痛、嗳气、反酸、嘈杂、恶心呕吐、便溏、便秘、食少纳呆、肢体困重、胸闷、喜太息、胁肋胀痛及小便短黄等症状改善方面,两组比较差异有统计学意义。在各主证组中,肝胃不和证治疗组在缓解胃脘胀满、嗳气、反酸、嘈杂、食少纳呆及胁肋胀痛症状上均较对照组有显著疗效(P<0.05~0.01);脾虚气滞证组在治疗胃脘疼痛及嗳气时,治疗组较对照组效果显著(P<0.01);脾胃虚弱证组患者中,治疗组较对照组可显著缓解胃脘疼痛及便溏症状(P<0.05~0.01)。结论柴术消痞汤是治疗胃痞病的有效经验方,与对照组相比,能明显改善胃脘胀满、嗳气、食少纳呆等临床症状。

Objective To study the clinical curative effect of Chaizhu Xiaopi Decoction on stomach bloating and provide the new prevention and treatment of stomach bloating by Chinese medicine.Methods A total of 120 patients diagnosed as stomach bloating were randomly allocated into modified Chaizhu Xiaopi Decoction group(n=60)and domperidone group(n=60).In the treatment group,patients were treated with Modified Chaizhu Xiaopi Decoction.In the control group,patients were treated with domperidone(10 mg,three times a day orally),and all patients were treated for four weeks.One month later we started to follow up and compared the difference of curative effect.Results After statistics,the efficiency of Chaizhu Xiaopi Decoction group was significantly higher than that of the control group(93.33%vs.72.88%,P<0.01).The symptomatic improvementsof the gastric fullness,gastric pain,belching,reflux,gastric upset,nausea,vomiting,loose stool,constipation,bad appetite,limb weakness,chest distress and so on were different statistically.At subgroup analysis,liver-stomach disharmony group had good effects on symptoms improvement(gastric fullness,gastric pain,belching,bad appetite)statistically(P<0.05~0.01),spleen deficiency and Qi stagnation group had good effects on symptom improvement(gastric pain,belching)(P<0.01),deficiency of spleen and stomach had good effects on symptom improvement(gastric pain,loose stool)(P<0.05~0.01).Conclusion Clinical research proved that Chaizhu Xiaopi Decoction was superior to control group in improving principal syndromes,such as gastric fullness and distention,belching,anorexia and other symptoms.
