加味八珍止眩汤改善颈性眩晕患者颈椎微循环及氧化应激损伤临床研究

Clinical Study on Modified Bazhen Zhixuan Tang in Improving Cervical Microcirculation and Oxidative Stress Injury in Patients with Cervical Vertigo

目的:研究加味八珍止眩汤治疗颈性眩晕对改善患者颈椎微循环、氧化应激损伤的影响。方法:选取济源市中医院2015年12月-2018年10月收治的颈性眩晕患者94例,根据治疗方法不同分为2组。对照组给予常规西药治疗,观察组在常规西药基础上给予加味八珍止眩汤治疗,分析2组患者治疗后的临床疗效。结果:2组治疗前症候积分等级情况、氧化应激指标、降钙素基因相关肽(CGRP)、左侧和右侧椎动脉、基底动脉的平均血流速度(TAMn)、收缩期最大血流速度(PS)差异均无统计学意义(P>0.05)。与治疗前比较,2组治疗后症候积分等级改善,超氧化物歧化酶(SOD)、CGRP水平及左侧和右侧椎动脉、基底动脉的PS上升(P<0.05),MDA水平下降(P<0.05);观察组左侧和右侧椎动脉、基底动脉的TAMn水平上升(P<0.05)。治疗后与对照组比较,观察组症候积分等级情况较优(P<0.05);恶心消失时间、呕吐消失时间、头痛消失时间及眩晕消失时间较短(P<0.05);SOD、CGRP水平、左侧和右侧椎动脉、基底动脉的TAMn、PS水平较高(P<0.05),丙二醛(MDA)水平较低(P<0.05)。结论:加味八珍止眩汤治疗可加快颈性眩晕患者临床症状的消失,改善颈椎微循环,减轻氧化应激损伤。

Objective:To study the effect of modified Bazhen Zhixuan tang on the improvement of cervical microcirculation and oxidative stress injury in patients with cervical vertigo. Methods: A total of 94 cases of patients with cervical vertigo admitted to Jiyuan Hospital of Chinese Medicine from December 2015 to October 2018 were divided into two groups according to their different treatment methods. The control group was treated with routine western medicine, and the observation group was additionally treated with modified Bazhen Zhixuan tang based on the treatment of the control group.After treatment,the clinical effect in the two groups was analyzed. Results:Before treatment,compared with the levels of symptom scores and calcitonin gene-related peptide(CGRP), the oxidative stress parameters, the time-average mean(TAMn) velocity of blood flow and peak systolic(PS) velocity of left and right vertebral artery and basilar artery in the two groups, there were no significance in the difference(P>0.05). Compared with those before treatment, the levels of symptom scores in the two groups were improved;the levels of superoxide dismutase(SOD),CGRP and the PS of left and right vertebral artery and basilar artery were increased(P<0.05);the levels of MDA were decreased. In the observation group, the levels of TAMn of left and right vertebral artery and basilar artery were increased when compared with those before treatment(P<0.05). After treatment,compared with those in the control group,the level of symptom scores in the observation group was better(P<0.05);the disappearance time of nausea,vomiting,headache and vertigo was earlier(P<0.05);the levels of SOD,CGRP,and TAMn and PS of left and right vertebral artery and basilar artery were higher(P<0.05),and the level of malondialdehyde(MDA) was lower(P<0.05). Conclusion:Modified Bazhen Zhixuan tang can accelerate the disappearance of clinical symptoms in patients with cervical vertigo, improve the cervical microcirculation, and reduce oxidative stress injury.
