摘要
目的:收集2008-2017年广州市某中医院的死亡病例数据,分析不同期间死亡病例的年龄构成、死因顺位及其变化趋势。方法:运用Excel 2007和SPSS20.0进行数据汇总和统计分析。分析2008-2017十年间死亡病例的死因顺位、年龄构成及两个期间死因顺位和年龄构成比的变化趋势。结果:该中医院2008-2017年10年间总死亡人数为5 801人,其中男3 527人,占60.80%;女2 274人,占39.20%;男女性别比例为1.55∶1。2008-2012年与2013-2017年各年龄组死亡人数的构成比差异有统计学意义(χ^2=36.401,P<0.05)。2008-2017年该院前5位的死因依次为肿瘤、循环系统疾病、呼吸系统疾病、内分泌营养和代谢疾病、损伤中毒和外因,前5位死因的人数占总体死亡人数的88.71%。2008-2012年60~79岁与80岁以上的死因构成比差异有统计学意义(χ^2=174.105,P<0.05)。2013-2017年60~79岁与80岁以上的死因构成比差异有统计学意义(χ^2=192.520,P<0.05)。结论:80岁以上死亡病例构成比逐年增加,肿瘤、循环呼吸系统死因占比上升,一方面原因可能是人们生活水平和老年人保健意识的提高,另一方面可能是由于医院的服务体系和诊疗技术的逐步完善。因此有必要倡导健康生活方式,做好健康教育,提高人群健康水平。
Objective:To collect the data of death cases in a hospital of Chinese medicine in Guangzhou from 2008 to2017,and analyze the composition of age,sequence of cause of death and its change trend at different periods. Methods:Excel 2007 and SPSS 20.0 were used for data summary and statistical analysis. The sequence of cause of death,composition of age and the change trend of them at two specific periods between 2008 and 2017 were analyzed. Results: The total number of deaths in the hospital from 2008 to 2017 was 5 801,including 3 527 male(60.80%) and 2 274 female(39.20%);the ratio of male to female was 1.55∶ 1. There was significant difference in the composition of deaths in each age group between 2008-2012 and 2013-2017(χ^2=36.401,P<0.05). From 2008 to 2017,the top five causes of death in this hospital were tumor,circulatory system diseases,respiratory system diseases,endocrine nutrition and metabolic diseases,injury and poisoning,and external causes,accounting for 88.71% totally. There was significant difference in the composition of cause of deaths between 60~79 years old and over 80 years old from 2008 to 2017(χ^2=174.105, P<0.05). There was significant difference in the composition of cause of deaths between 60~79 years old and over 80 years old from 2008 to 2012(χ^2=192.520, P<0.05). Conclusion: The proportion of death cases over 80 years old is increasing year by year, and the composition of death cases caused by tumors and circulatory and respiratory system diseases increases. On the one hand,this may be due to the improvement of people’s living standards and the awareness of health care of the elderly;on the other hand,it may be because of the gradual improvement of the service system and diagnosis and treatment technology of the hospital. Therefore, it is necessary to advocate a healthy lifestyle, promote health education and improve the health of people.
