摘要
目的:观察选择性头部亚低温治疗新生儿缺氧缺血性脑病(HIE)的临床效果。方法:选取46例HIE患儿作为研究对象,依据随机数字表法分为对照组和观察组各23例,对照组采取常规对症治疗,观察组在对照组的基础上采取选择性头部亚低温治疗,比较治疗后两组神经功能、运动和智力发育情况及不良反应发生率。结果:出生后7 d、14 d、28 d,观察组新生儿一般反应、原始反射、主动肌张力、被动肌张力、行为能力等评分明显高于对照组,出生后3个月、6个月运动发育指数与智能发育指数明显高于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:选择性头部亚低温治疗新生儿HIE的临床效果较显著,能有效提高新生儿的神经功能、运动和智能水平,且安全性高。
Objective:To observe clinical effects of selective head mild hypothermia treatment on neonatal hypoxic ischemic encephalopathy(HIE).Methods:46 neonates with hypoxic ischemic encephalopathy were selected as the research objects,and were divided into control group(n=23)and observation group(n=23)according to the random number table method.The control group was treated with conventional symptomatic treatment,while the observation group was given selective head mild hypothermia treatment based on that of the control group.Then,the neurological function,motor and mental development and the incidence of adverse reactions were compared between the two groups after the treatment.Results:The scores of general reaction,primitive reflex,active muscle tone,passive muscle tone,and behavioral capacity of the neonates of 7d,14d and 28d in the observation group were significantly higher than those in the control group;the motor and mental development indexes of the neonates of 3 and 6 months were also significantly higher than those in the control group;and the differences were statistically significant(P<0.05).Further,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Selective head mild hypothermia treatment for neonatal hypoxic ischemic encephalopathy has a significant clinical effect.It can effectively improve the neurological function and promote the motor and mental development levels with high safety.
