浅谈某院静脉用药调配中心药师在不合理用药现状中的作用及建议

Brief discussion of current situations and recommendations of irrational drug use of pharmacists in pharmacy intravenous admixture service of a hospital

目的:研究郑州大学附属儿童医院静脉用药调配中心(PIVAS)药师在不合理用药现状中的作用及建议。方法:回顾性分析2019年1月至2019年4月郑州大学附属儿童医院药学部接收的2564条输液医嘱,统计其中的不合理医嘱条数,分析不合理医嘱的原因,并提出建议。结果:2564条输液医嘱中存在83条不合理医嘱,所占比例为3.24%;经分类统计后,发现导致不合理医嘱的原因主要有药品配伍有误(27条,32.53%)、溶媒选择不当(35条,42.17%)、给药剂量不当(21条,25.30%)。结论:药品配伍有误、溶媒选择不当、给药剂量不当是临床常见的不合理用药问题,但通过加强PIVAS药师医嘱审核,可以减少用药错误,确保用药安全性。

Objective:To study current situations and suggestions of irrational drug use of pharmacists in pharmacy intravenous admixture service(PIVAS)of the Children’s Hospital Affiliated to Zhengzhou University.Methods:A retrospective analysis was performed on 2,564 infusion orders received from the department of pharmacy of the children’s hospital of Zhengzhou university from January 2019 to April 2019.The irrational doctor orders were statistically analyzed.Then,the causes of the irrational doctor orders were analyzed in order to make recommendations.Results:There were 83 irrational doctor orders among the 2564 cases,accounting for 3.24%.The classified statistics analysis showed that the main causes of irrational medical orders were incorrect drug compatibility(27 cases,32.53%),improper choice of solvent(35 cases,42.17%)and improper dosage(21 cases,25.30%).Conclusions:Incorrect drug compatibility,improper selection of solvents and improper dosage are the common irrational drug use problems in the clinic.However,by strengthening the review of PIVAS pharmacist orders,medication errors can be reduced and drug safety can be ensured.
