腹腔镜与开放无张力疝修补术的临床疗效比较

Comparison of Clinical Efficacy between Laparoscopic Inguinal Hernia Repair and Open Tension-free Herniorrhaphy

目的:对比腹腔镜下腹股沟疝无张力修补术与开放无张力疝修补术治疗腹股沟疝疗效及预后。方法:对我院于2015年6月-2018年6月收治的共89例腹股沟疝患者的临床资料进行回顾性分析,观察组(45例)和对照组(44例)分别为两种不同的手术方法,观察组行完全腹膜外修补术(TEP),对照组行开放式无张力疝修补术,比较两组患者的临床疗效及复发情况。结果:观察组手术时间、术后恢复正常活动时间、住院时间显著短于对照组相应指标(P<0.05);观察组术中出血量少于对照组相应指标(P<0.05);观察组住院费用显著高于对照组(P<0.05);腹股沟疝手术后常见并发症为切口并发症、阴囊并发症、尿潴留等,观察组并发症发生率低于对照组(P<0.05);观察组复发率明显低于对照组,差异无统计学意义(P>0.05)。结论:两种术式均能够安全有效地治疗腹股沟疝,但各有优缺点;相比而言,腹腔镜疝修补术具有手术时间短、创伤小、术后恢复快等优点,然而,却存在费用较高、手术难度大、心肺功能要求严格等劣势,故应遵循个体化的标准,根据患者具体情况来决定手术方式。

Objective:To compare clinical effect of laparoscopic inguinal hernia repair and traditional tension-free herniorrhaphy.Methods:89 cases of inguinal hernia patients who accepted treatments in our hospital during June 2015 and June 2018 were selected,and they were divided into the observation group(45 cases)and the control group(44 cases)by the different methods of operation.In the observation group patients were given laparoscopic inguinal hernia repair for treatment,while in the control group patients were treated with open tension-free hernia repair treatment,Then comparing the clinical efficacy and complications of the two groups of patients.Results:The operation time,time to resume normal activities after surgery and hospital stay time in the observation group were significantly shorter than that of the control group(P<0.05);The amount of bleeding in the observation group was significantly less than that of the control group(P<0.05);The hospitalization expenses in the observation group was more expensive than that of the control group(P<0.05).The common complications after the operation are from incision,scrotum and bladders and the incidence rate in the observation group was lower than that of the control group(P<0.05).The recurrence rate in the observation group was lower than that of the control group(P>0.05).Conclusions:Two types of surgery can both cure the inguinal hernia patients safely and effectively,While they have respective advantages and disadvantages.Inguinal hernia repair has the advantages of short operation time,small injuries and fast recovery.However,it has the disadvantages of expense cost and difficult operation.Therefore,we should obey the individualized standard and choose the appropriate operation methods by the specific situation of patients.
