血清降钙素原和hs-CRP在早期新生儿感染性疾病应用价值的Meta分析 被引量:14

Meta-analysis of the application value of serum procalcitonin and hs-CRP in early neonatal infectious diseases

摘要 目的探讨血清降钙素原(PCT)和超敏C-反应蛋白(hs-CRP)在早期新生儿感染性疾病中的应用价值,以便为临床早期诊断提供参考价值。方法检索Medline、Web of Science、The Cochrane Library、VIP、WanFang Data和CNKI数据库,查找有关血清PCT和hs-CRP在早期新生儿感染性疾病中应用价值的相关研究,检索时限截至2018年11月15日,由两位评价员按纳入与排除标准独立筛选文献、提取资料并评价文献质量,运用Stata 12.0软件进行Meta分析。结果最终纳入文献12篇,结果显示,研究组中PCT浓度高于对照组,差异有统计学意义[SMD=4.508,95%CI(3.580,5.436),P<0.001],hs-CRP浓度高于对照组,差异有统计学意义[SMD=4.995,95%CI(3.778,6.211),P<0.001]。结论在新生儿发生感染性疾病时,血清PCT和hs-CRP会升高,因此PCT和hs-CRP指标的测定,对及时发现新生儿感染性疾病具有重要意义,有望为临床上快速诊断新生儿感染性疾病提供重要的参考。 OBJECTIVE To investigate the value of serum procalcitonin(PCT)and high-sensitivity C-reactive protein(hs-CRP)in early neonatal infectious diseases in order to provide reference value for early clinical diagnosis.METHODS Relevant literature search of Medline,Web of Science,The Cochrane Library,VIP,WanFang Data,and CNKI databases was conducted for serum PCT and hs-CRP that assessed the application value in early neonatal infectious diseases inclusive to Nov.15,2018.Two reviewers independently screened the literature,extracted the data and evaluated the quality of the literature according to the inclusion and exclusion criteria,and performed meta-analysis using Stata 12.0 software.RESULTS Finally,12 articles were included in the literature.The results showed that the PCT concentration in the study group was significantly higher than that in the control group[SMD=4.508,95%CI(3.580,5.436),P<0.001],and the hs-CRP concentration was significantly higher than that in the control group[SMD=4.995,95%CI(3.778,6.211),P<0.001].CONCLUSION In the case of neonatal infectious diseases,serum PCT and hs-CRP will increase.Therefore,the determination of PCT and hs-CRP indicators is of great significance for the timely detection of neonatal infectious diseases,and is expected to provide an important reference for rapidly diagnosis of neonatal infectious diseases in clinical practice.
