蝮蛇解毒汤对蝮蛇咬伤患者急性肾损伤因子NGAL的影响

Effect of Viper Detoxifying Decoction on The NGAL Level of Viper Bite Patients with Acute Kidney Injury

目的:探讨蝮蛇解毒汤对蝮蛇咬伤病人早期肾损伤相关因子-中性粒细胞明胶酶相关脂质运载蛋白(neutrophil gelatinase-associated lipocalin,NGAL)水平的影响。方法:收集入住我院80例蝮蛇咬伤早期出现NGAL水平异常患者,按随机数字表法分为试验组和对照组各40例,2组均采用针对蝮蛇咬伤的基础治疗,试验组加用蝮蛇解毒汤口服,于治疗第1、3、6天检测2组患者的血浆中性粒细胞明胶酶相关脂质运载蛋白(pNGAL)、尿液中性粒细胞明胶酶相关脂质运载蛋白(uNGAL)和血清肌酐(serum creatinine,Scr)水平。结果:对照组治疗前后Scr未有明显下降,而试验组在第6天下降,差异有统计学意义;对照组第3天pNGAL、uNGAL下降,差异无统计学意义,而试验组在治疗第3、6天pNGAL、uNGAL下降,差异有统计学意义,且试验组下降在治疗第3天、第6天与对照组比较差异有统计学意义。结论:应用中药蝮蛇解毒汤能降低蝮蛇咬伤患者的Scr、pNGAL和uNGAL水平,对蝮蛇咬伤患者早期的急性肾损伤有预防和治疗作用。

Objective:To investigate the effect of viper detoxifying decoction on NGAL level in patients with early kidney injury after Agkistrodon bite. Methods:Collecting 80 cases of hospitalized patients with the viper bites in our hospital, randomly divided into 2 groups(control group 40 cases&experimental group 40 cases), Both groups received basic treatment for viper bite,experimental group add viper detoxifying decoction,Serum creatinine(Scr),pNGAL and uNGAL were detected on the 1 st, 3 rd and 6 th days after treatment. Results:There was no significant difference in Scr in the control group before and after treatment, while there was significant difference in the experimental group on the 6 th day of treatment.The uNGAL and pNGAL levels in the control group did not decrease significantly at the 3 rd days of treatment,while the levels of uNGAL and pNGAL in the experimental group decreased significantly on both 3 rd and 6 th days after treatment;And the decrease of pNGAL and uNGAL in the experimental group was statistically significant on both the 3 th and 6 th days of treatment. Conclusion:The application of Chinese medicine viper detoxification decoction can reduce Scr,pNGAL and uNGAL levels in viper bite patients, and can prevent and treat early acute renal injury in viper bite patients.
