摘要
目的:系统评价火针治疗中风后痉挛性瘫痪(spastic paralysis after stroke, SPAS)的疗效与安全性。方法:计算机检索CNKI、万方、CBM、VIP、PubMed及Cochrane library数据库中有关火针治疗SPAS的随机对照试验(randomized controlled trials,RCTs),检索时间为建库至2018年2月,并采用REVMAN5.3软件行Meta分析。结果:最终纳入14篇RCTs共918例患者,Meta分析结果显示,火针组在总有效率、治愈率、Fugl-Meyer运动功能评分、Barthel指数评分、改良Ashworth痉挛量表评分、临床痉挛指数评分和神经功能缺损评分方面均优于对照组。结论:火针治疗SPAS疗效肯定且安全性可靠,由于本研究纳入文献质量低、样本小,仍需高质量和大样本的RCTs加以验证。
Objective: To review systematically the clinical effects and safety of fire needle therapy for spastic paralysis after stroke(SPAS). Methods: We searched six electronic databases to collect randomized controlled trials(RCTs) of fire needle therapy for SPAS published before February 2018. The RevMan 5.3 software was used for meta-analysis. Results: 14 RCTs involving 918 cases were included. The results of Meta-analysis showed that Fire needle therapy group was superior to the control group in improving total effective rate, cure rate, Fugl-Meyer Motor Function Rating Scale, Barthel Index, Modified Ashworth Spasm Scale, Clinical Spasm Index and Neurological Deficit Score. Conclusion: Fire needle therapy is an effective and safe treatment for SPAS. Due to the low quality and small sample size of the included studies, more rigorously designed RCTs with high quality and large-scale are recommended in future.
