摘要
目的探讨"连续性代谢综合征评分"(continuous metabolic syndrome score,c MetS)对新疆15~18岁人群代谢综合征(metabolic syndrome,MetS)筛查价值,为确定c MetS对不同性别最佳截断点提供参考。方法分层整群抽取新疆13座城市15~18岁人群共4745名。c MetS是按年龄和性别进行计算标准化腰围、平均动脉压、高密度脂蛋白、三酰甘油、空腹血糖的Z分,再对单个变量的Z分进行累加计算。结果4745名城市人群中,有16.3%为超重,5.2%属于肥胖,其中男性超重和肥胖比例(18.1%,13.8%)高于女性(6.0%,4.1%)(χ^2值分别为15.36,7.89,P值均<0.05)。总MetS患病率为6.1%,男性和女性分别为6.6%,5.4%。c MetS与MetS组分之间具有相关性,平均c MetS值随着MetS组分增加而增加(P<0.05)。c MetS总截断值为0.99(灵敏度为68.0%,特异度为80.7%),ROC曲线下面积为84.9%。男生和女生的c MetS评分分别为0.80和1.48。结论与MetS相比,c Mets在筛查不同性别人群患MetS中是一种更灵敏的代谢变化标志物。
Objective The purpose of the present study was to evaluate of continuous metabolic syndrome score(c MetS in screening metabolic syndrome(MetS)and to determine the cut-off values in a representative sample of Xinjiang population aged15 to 18 years old.Methods A stratified cluster sampling was used to select participants aged 15-18 years from 13 cities in Xinjiang.c MetS was calculated by summing up the Z-scores of standardized waist circumference,mean arterial pressure,high-density lipoprotein,triglyceride,and fasting blood glucose by age and gender.Results Totally 16.3%of subjects were overweight,and 5.2%were obese.The proportion of overweight and obesity in males was significantly higher than that in females(18.1%vs 13.8%)(6.0%vs.4.1%)(χ^2=15.36,7.89,P<0.05).The prevalence of total MetS was 6.1%,with 6.6%and 5.4%for men and women,respectively.There was a correlation between c MetS and MetS components,and the average c MetS value increased with increasing MetS component(P<0.05).The total cut-off value of c MetS was 0.99(sensitivity 68.0%,specificity 80.7%),and the area under the ROC curve was 84.9%.The c MetS scores for boys and girls were 0.80 and 1.48,respectively.Conclusion Compared with MetS,c MetS shows more accuracy in screening MetS among youth aged 15-18 years in both gender.
