破格救心汤对心肌梗死大鼠室性心律失常发生与心功能的影响 被引量:8

Effects of Poge Jiuxin Decoction on Ventricular Arrhythmia and Cardiac Function in Rats with Myocardial Infarction

摘要 目的探讨破格救心汤对心肌梗死后大鼠室性心律失常发生与心功能的影响。方法选用SD大鼠,通过冠脉结扎方法构建大鼠心肌梗死模型,随机分为假手术组、模型组、破格救心汤组。手术后4周,检测大鼠心功能,诱发室性心律失常并进行评分,酶联免疫吸附法(ELISA)检测血清中神经生长因子(NGF)水平,HE和Masson染色观察各组心肌形态情况。结果对各组大鼠心肌组织切片病理观察发现,假手术组大鼠心肌组织结构正常,心肌细胞形态完整,心肌纤维排列整齐,胞浆纹理清晰;模型组心肌细胞出现变形、破裂、坏死,心肌纤维排列紊乱,部分出现断裂、分离,同时伴有炎性细胞浸润;破格救心汤组心肌细胞亦出现变形、断裂、坏死,但心肌纤维断裂现象较模型组明显减轻。对各组大鼠心功能进行分析发现,与假手术组相比较,模型组及破格救心汤组大鼠EF明显降低,LVEDD及LVESD明显升高(均P<0.05);破格救心汤组大鼠EF明显高于模型组,LVEDD及LVESD明显低于模型组(均P<0.05)。通过对各组大鼠进行室性心律失常评分发现,与假手术组相比较,模型组与破格救心汤组室性心律失常评分明显升高(均P<0.05),而破格救心汤组室性心律失常评分较模型组降低(均P<0.05)。经检测各组大鼠血清中NGF水平发现,与假手术组相比较,模型组与破格救心汤组NGF水平明显升高,而破格救心汤组血清中NGF含量较模型组减少(均P<0.05)。结论破格救心汤能改善心肌梗死后大鼠心脏功能,减少室性心律失常发生,并能减少心梗后大鼠血清中NGF含量,其机制可能与减少心肌梗死后神经生长因子分泌相关。 Objective:To investigate the effects of Poge Jiuxin Decoction on ventricular arrhythmia and cardiac function in rats with myocardial infarction.Methods:SD rats were used as the research object.The rat model of myocardial infarction was established by coronary artery ligation.They were randomly divided into sham operation group,myocardial infarction group and myocardial infarction+Poge Jiuxin Decoction group.4 weeks after surgery,cardiac function was detected by echocardiography and programmed electrical stimulation was performed to evaluate the severity of ventricular arrhythmias.The serum levels of nerve growth factor were detected by ELISA.Histochemical staining was performed to observe the myocardial morphology of each group.Results:The pathological observation of myocardial tissue sections of rats in each group showed that the myocardial tissue structure of the rats in the sham operation group was normal;the morphology of the cardiac cells was intact;the myocardial fibers were arranged neatly,and the cytoplasmic texture was clear.The myocardial cells in the myocardial infarction group were deformed,ruptured and necrotic.Myocardial fibers were disordered,partially broken and separated,and accompanied by inflammatory cell infiltration.Myocardial cells in the myocardial infarction+Poge Jiuxin Decoctiongroup also showed deformation,fracture and necrosis,but myocardial fibrosis was significantly less than that in the myocardial infarction group.Analysis of cardiac function in each group of rats showed that EF was significantly decreased in myocardial infarction group and myocardial infarction+Poge Jiuxin Decoction;LVEDD and LVESD were significantly increased(P<0.05).Myocardial infarction+Poge Jiuxin Decoction group rats EF was significantly higher than myocardial infarction group,LVEDD and LVESD were significantly lower than myocardial infarction group(both P<0.05).Ventricular arrhythmia scores were analysised in each group.Compared with the sham-operated group,the ventricular arrhythmia scores in the myocardial infarction group and the myocardial infarction+Poge Jiuxin Decoction group were significantly increased,while the myocardial infarction+Poge Jiuxin Decoctiongroup was lower than that of the myocardial infarction group(both P<0.05).The levels of NGF in the serum were significantly higher in the myocardial infarction group and myocardial infarction+Poge Jiuxin Decoction group,while the myocardial infarction+Poge Jiuxin Decoction group was lower than that in the myocardial infarction group.Conclusion:Poge Jiuxin Decoction can improve the heart function of rats with myocardial infarction,reduce the occurrence of ventricular arrhythmia,and reduce the serum nerve growth factor content.Therefore,the mechanism may be related to the reduction of nerve growth factor secretion after myocardial infarction.
