外周血白细胞总数及其亚群比值对判断上皮性卵巢癌预后的价值

Value of Total White Blood Cell Count and Its Subgroup Ratio in Peripheral Blood in Predicting Prognosis of Epithelial Ovarian Cancer

目的探讨外周血白细胞总数及其亚群比值对判断上皮性卵巢癌(EOC)患者预后的价值。方法回顾性分析108例EOC患者初治前血常规值、无进展生存期(PFS)、总生存期(OS)资料,记录白细胞(WBC)值、单核细胞/淋巴细胞比值(MLR)、中性粒细胞/淋巴细胞比值(NLR)、中性粒细胞×单核细胞/10^4比值(MNM),并绘制ROC曲线,确定MLR、NLR、MNM、WBC最佳截点并将其分别分为高、低值组,分析各观察指标与PFS和OS的关系。结果 ROC曲线:MLR、 NLR、MNM、WBC最佳截点依次为0.218、2.36、0.000 202 075、6.645。单因素检验:初治前MLR、NLR、WBC高值与EOC患者术后PFS低、OS短相关(P<0.05)。多因素分析:高MLR值与PFS低、OS短均密切相关、高NLR值仅与PFS低关系密切(P<0.05)。结论初治前高MLR、NLR、MNM及WBC值对EOC患者预后评估均具有一定价值,以高MLR值评估价值最佳。

Objective To investigate the value of total white blood cell(WBC) count and its subgroup ratio in the prognosis of epithelial ovarian cancer(EOC).Methods Data of 108 patients with EOC were retrospectively analyzed,including blood routine indexes,progression-free survival(PFS),overall survival,WBC count,monocyte/lymphocyte ratio(MLR),neutrophils/lymphocyte ratio(NLR) and neutrophils × monocytes/10^4 ratio(MNM).ROC curves were drawn to determine the optimal cut-off points of MLR, NLR,MNM and WBC,which were divide into high and low value groups to analyze the relationships between these indexes and PFS and OS.ResultsThe optimal cut-off points of MLR,NLR,MNM and WBC were 0.218,2.36,0.000 202 075 and 6.645,respectively.Univariate analysis showed that the high MLR,NLR and WBC before initial treatment were associated with long PFS and short OS after treatment in EOC patients(P<0.05).Multivariate analysis showed that high MLR was closely correlated with short PFS and OS,while high NLR value was only related to short PFS(P<0.05).Conclusion The high levels of MLR,NLR,MNM and WBC before primary treatment,especially high MLR,are of certain value for prognostic evaluation in EOC patients.
