摘要
目的:应用二维斑点追踪技术评估高尿酸血症患者左房功能与心外膜脂肪厚度的相关性。方法:选取初次诊断高尿酸血症患者40例,另外选取健康志愿者30例作为对照组,应用二维斑点追踪技术获得心室收缩期左心房整体峰值纵向应变(LAS-s)、舒张早期左房纵向应变(LAS-e)、舒张晚期左房纵向应变(LAS-a),比较两组之间的差异性,分析高尿酸血症组心外膜脂肪厚度与左心房整体心肌纵向应变功能的相关性。结果:与对照组比较,高尿酸血症组心外膜脂肪厚度显著增厚(P<0.05),同时与对照组比较,高尿酸血症组收缩期左房整体峰值纵向应变(LAS-s)、舒张早期左房纵向应变(LAS-e)均减低(P<0.05),舒张晚期左房纵向应变增高(LAS-a)(P<0.05);与对照组比较,高尿酸血症组左房内径增大,体重指数增加,E/e’比值增加,比较差异有统计学意义(P均<0.05)。相关性分析显示,高尿酸血症组心外膜脂肪厚度与心室收缩期左房整体峰值纵向应变值(LAS-s)、舒张早期左房纵向应变(LAS-e)呈负相关性(r=-0.63,r=-0.56,P均<0.05)。结论:高尿酸血症患者心外膜脂肪厚度增厚,高尿酸血症患者心外膜脂肪厚度与左房心肌功能具有密切相关性。
Objective:To evaluate the correlation between left atrial function and epicardial adipose tissue thickness in patients with hyperuricemia by two-dimensional speckle tracking technique. Methods:40 patients with hyperuricemia were selected as hyperuricemia group,and 30 healthy volunteers were selected as control group. The two-dimensional speckle tracking technique was used to obtain the overall peak longitudinal strain of the left atrium during ventricular systole(LAS-s),left atrial longitudinal strainduring early diastole(LAS-e),and left atrial longitudinal strain during late diastole(LAS-a). The differences between the two groups were compared,and the correlation between epicardial adipose tissue thickness and overall left atrial longitudinal strain function was analyzed. Results:Compared with the control group,the thickness of epicardial adipose tissue of the hyperuricemia group was significantly increased(P<0.05);the LAS-s and LAS-e of the hyperuricemia group were all significantly decreased(P<0.05),while LAS-a was increased(P<0.05);the left atrial diameter,body mass index and E/e’ratio of the hyperuricemia group were increased(All P<0.05). Correlation analysis showed that epicardial adipose tissue thickness in the hyperuricemia group was negatively correlated with LAS-s(r=-0.63,P<0.05) and LAS-e(r=-0.56,P<0.05). Conclusion:The thicker the epicardial adipose tissue thickness,the more serious the damage of left atrial function. The epicardial adipose tissue thickness is closely related to left atrial function in hyperuricemia patients.
