摘要
目的分析就诊儿童患者抗核抗体谱阳性分布趋势。方法采用间接免疫荧光法(IIF)为ANA筛查试验、免疫印迹法(LIA)为ANAs确认试验,检测1544例就诊患儿的抗核抗体谱。结果1544例患儿IIF-ANA总阳性率为8.5%,其中AID组阳性率(67.8%)明显高于对照组(6.2%)(P<0.05)。女性患儿IIF-ANA阳性率(10.7%)明显高于男性患儿(6.9%)(P<0.05),且以女性11岁~16岁最高(24.6%)(P<0.05)。AID组中,IIF-ANA、抗Sm、抗组蛋白、抗ds DNA、抗NUC阳性率随患儿年龄的增加而升高(P<0.05),而其他几种自身抗体在不同年龄患儿中的阳性率差异无统计学意义(P>0.05)。IIF-ANA^+/LIA-ANAs^+组83例,IIF-ANA^+/LIA-ANAs^-组49例,IIF-ANA^-/LIAANAs^-组1412例,IIF-ANA^-/LIA-ANAs^+组46例。结论就诊儿童患者IIF-ANA阳性率存在性别和年龄差异,多见于年长的女性患儿。抗核抗体谱中部分自身抗体随患儿年龄的增加而升高。IIF-ANA与LIA-ANAs中的自身抗体多见于AID患儿,但其他疾病也会出现,实验室最好同时进行筛查试验和确认试验。
Objective To analyze the positive distribution trend of antinuclear antibodies spectrum(ANAs)in pediatric patients.Methods A total of 1544 clinical patients’serum samples were detected by indirect immunofluorcscence(IIF)for screening assay of ANA,and the antinuclear antibody spectrum of 1544 children were detected.Results The positive rate of ANA in 1544 patients was 8.5%.The positive rate of ANA was statistically higher in AID group(67.8%)than that in control group(6.2%,P<0.05).The positive rate of ANA was statistically higher in female(10.7%)than that in male(6.9%,P<0.05).The group of 11-16 years old in female had the highest positive rate of ANA(24.6%,P<0.05).The positive rates of ANA,anti-Sm,anti-histone,anti-ds DNA and anti-NUC antibodies increased with age(P<0.05).There was no statistical significance on the difference in the positive rates of other antibodies(P>0.05).It was identified that 83 cases were IIF-ANA^+/LIA-ANAs^+,49 cases were IIF-ANA^+/LIA-ANAs^-,1412 cases were IIF-ANA^-/LIA-ANAs^-,and 46 cases were IIF-ANA^-/LIA-ANAs^+.Conclusion The positive rate of ANA is different in ages and sexes and higher in elder female children.The positive rates of some antibodies increase with age.Some autoantibodies in the antinuclear antibody spectrum increase with the age of child.The autoantibodies in IIF-ANA and LIA-ANAs are more common in children with AID,but other diseases also occur.It is better to conduct screening tests and confirmation tests at the same time in the laboratory.
