血清外泌体miR-184在甲状腺乳头状癌中的表达及其诊断效能

Expression of serum exosomes miR-184 in papillary thyroid carcinoma and its diagnostic value

目的探讨血清外泌体miR-184在甲状腺乳头状癌中的表达值及其意义。方法检测410例甲状腺乳头状癌患者、100例结节性甲状腺肿患者、100例体检健康者的血清外泌体中miR-184水平,评价血清外泌体miR-184诊断甲状腺乳头状癌的敏感性和特异性。结果甲状腺乳头状癌组、结节性甲状腺肿组及体检健康组的血清外泌体miR-184的相对表达量比较差异有统计学意义(P<0.05),其中甲状腺乳头状癌患者血清中的外泌体miR-184水平最高;血清外泌体miR-184诊断甲状腺乳头状癌的敏感性为91.70%,特异性为82.45%,约登指数为0.748,其曲线下面积为0.829(95%CI:0.704~1.032)。结论甲状腺乳头状癌患者血清外泌体中miR-184呈高表达,其诊断敏感性及特异性较高,或将成为甲状腺乳头状癌的临床诊断生物学标志物。

Objective To evaluate the expression of serum exosomes miR-184 in papillary thyroid carcinoma and its diagnostic value.Methods Firstly,the levels of miR-184 in the serum exosomes of 410 patients with papillary thyroid cancer,100 patients with nodular goiter and 100 subjects diagnosed healthy by physical examination were measured.Secondly,the sensitivity and specificity of serum exosomes miR-184 in the diagnosis of papillary thyroid carcinoma were evaluated.Results Statistically significant difference were found in the relative expression of serum exosomes miR-184 among the papillary thyroid carcinoma group,the nodular goiter group and the healthy group(P<0.05).The level of expression of exosomes miR-184 in serum of patients with thyroid papillary carcinoma was highest of the three groups.The sensitivity of serum exosomes miR-184 for the diagnosis of papillary thyroid carcinoma was 91.70%,and the specificity was 82.45%.The Youden index was 0.748,and the area under the curve was 0.829(95%CI:0.704~1.032).Conclusions The level of expression of miR-184 in serum exosomes of patients with papillary thyroid carcinoma is high.Its diagnostic sensitivity and specificity are high and hence can serve as biomarkers for clinical diagnosis of biomarker for papillary thyroid cancer.
