64排螺旋CT三期增强扫描对原发性肝癌的诊断价值分析

Diagnostic Value of 64-slice Spiral CT Three-phase Enhanced Scanning in Primary Hepatocellular Carcinoma

目的分析64排螺旋CT三期增强扫描对原发性肝癌的诊断价值。方法选取2016年2月至2019年4月在昆山市第四人民医院增强CT检出随访确诊的和已确诊复查患者的原发性肝癌患者30例,对其均行64排螺旋CT三期增强扫描诊断,观察本组患者的扫描结果。结果扫描检查显示,各类型原发性肝癌的检出率分别为:巨块型100.00%(6/6)、结节型80.00%(4/5)、巨块结节型80.00%(8/10)、弥漫型结节型100.00%(9/9);原发性肝癌的阳性检出率为90.00%(27/30),动脉期、静脉期、延迟期低密度检出率逐渐升高,高密度检出率逐渐降低,各时期差异有统计学意义(P<0.05)。结论临床上对原发性肝癌患者进行诊断时,采用64排螺旋CT三期增强扫描诊断,其能够根据肿瘤各期的影像学特征进行诊断,有较高的应用价值。

Objective To analyze the diagnostic value of 64-slice spiral CT three-phase enhanced scanning in primary hepatocellular carcinoma.Methods A total of 30 patients with primary hepatocellular carcinoma who were confirmed by follow-up and reexamined by enhanced CT from February 2016 to April 2019 in Kunshan Fourth People's Hospital were selected.All patients were diagnosed by three-phase enhanced 64-slice spiral CT,and the scanning results of these patients were observed.Results Scanning examination showed that the detection rates of all types of primary liver cancer were 100.00%(6/6)for massive type,80.00%(4/5)for nodular type,80.00%(8/10)for massive nodular type,and 100.00%(9/9)for diffuse nodular type,respectively.The positive detection rates of primary hepatocellular carcinoma were 90.00%(27/30).The detection rate of low density in arterial,venous and delayed phases gradually increased,high-density detection rate gradually decreased,the difference was significant among different phases(P<0.05).Conclusion In clinical diagnosis of primary hepatocellular carcinoma,64-slice spiral CT three-phase contrast-enhanced scan can be used to diagnose primary hepatocellular carcinoma according to the imaging characteristics of each stage of the tumor,which has a high application value.
