摘要
目的:探究血清Hcy对布加综合征患者肝脏纤维化的诊断效果研究,为临床提供诊断依据。方法:选取本院2016年7月~2018年7月收治的BCS患者94例作为BCS组,依据Child-Pugh分级,将患者分为Child-Pugh A级组48例(A组),Child-Pugh B级组30例(B组),Child-Pugh C级组16例(C组),同期选取31例健康体检者作为对照组。对各组纤维化指标及Hcy水平进行比较,对患者血清Hcy水平与肝纤维化指、Child-Pugh分级的相关性分析进行分析。结果:BCS组患者肝脏纤维化指标、血清Hcy水平均显著高于对照组,差异均有统计学意义(P<0.05);C组患者肝脏纤维化指标、血清Hcy水平较B组高,B组肝脏纤维化指标、血清Hcy水平较A组高,差异均有统计学意义(P<0.05);各指标随Child-Pug h分级升高而增加(P<0.05);对照组患者血清Hcy水平与肝脏纤维化各指标均无显著相关性(r=0.288、0.068、0.224,P>0.05);BCS组患者血清Hcy水平与肝脏纤维化各指标均呈正相关(r=0.464、0.461、0.531,P<0.05);BCS组患者的血清Hcy水平与Child-Pugh分级情况呈正相关(r=0.664,P<0.05)。ROC分析提示:HCY水平对肝脏纤维化有较高的诊断价值(P<0.05),ROC-AUC=0.829。结论:BCS患者发生肝脏纤维化时,其血清Hcy水平会发生显著提高,且患者Hcy平与肝脏纤维化程度相关性良好,可作为BCS患者肝脏纤维化情况的诊断指标。
Objective:To investigate the effect of serum Hcy on the diagnosis of hepatic fibrosis in patients with Budd-Chiari syndrome,and provide a basis for clinical diagnosis.Methods:A total of 94 patients with BCS admitted to our hospital from July 2016 to July 2018 were selected as the BCS group.According to Child-Pugh classification,the patients were divided into Child-Pugh A group with 48 cases(A group),Child-Pugh B group with 30 cases(B group),Child-Pugh C group with 16 cases(C group).At the same period,31 healthy subjects were selected as the control group.The fibrosis indexes and Hcy levels in each group were detected,and the correlation analysis between serum Hey levels,liver fibrosis index and Child-Pugh classification was performed Results:The liver fibrosis index and serum Hcy level in BCS group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The liver fibrosis indexes and serum Hcy levels in group C were higher than those in group B.The liver fibrosis indexes and serum Hcy levels in group B were higher than those in group A,and the differences were statistically significant(P<0.05).Each index increased with the Child-Pugh classification(P<0.05).No significant correlarion was found between serum Hcy level and liver fibrosis indexes in the control group(r=0.288,0.068,0.224,P>0.05).Serum Hcy levels in patients with BCS were positively correlated with liver fibrosis(r=0.464,0.461,0.531,P<0.05).Serum Hcy levels in patients with BCS were positively correlated with Child-Pugh classification(1^0.664,P<0.05).ROC analysis suggested that:HCY levels have a higher diagnostic value for liver fibrosis(P<0.05),ROC-AUC=0.829.Conclusion:Serum Hcy levels increased significantly for BCS patients with hepatic fibrosis,and the correlation between Hcy level and hepatic fibrosis is good,and it can be used as a diagnostic index of liver fibrosis in patients with BCS.
