128层CT多征象分析联合动态增强扫描对孤立性肺结节良、恶性鉴别诊断的价值

The Value of 128-Slice CT Multi-Sign Analysis Combined with Dynamic Enhancement Scan in Differential Diagnosis of Benign and Malignant Solitary Pulmonary Nodules

目的探究128层CT多征象分析联合动态增强扫描鉴别诊断孤立性肺结节良、恶性的应用价值。方法选取2016年7月至2019年7月宿迁市人民医院收治的孤立性肺结节患者40例,对全部患者采用128层CT多征象分析仪联合动态增强扫描鉴别,以手术病理检查或穿刺活检结果为判断标准。结果128层CT联合动态增强扫描检查孤立性肺结节良、恶性诊断的敏感性为95.24%,特异性为89.47%,准确性为92.50%。同类型良恶性肺结节患者CT增强值均比CT平扫值高,但是恶性肺结节CT值变化幅度更大(P<0.05)。恶性结节的PH值、孤立性肺结节强化峰值与主动脉强化峰值的比值(SPH/PPH)均显著大于良性结节(P<0.05)。结论128层CT多征象分析联合动态增强扫描鉴别诊断孤立性肺结节良恶性分辨率高,且检查方式简便快捷,得临床推广应用。

Objective To explore the application value of 128-slice CT multi-sign analysis combined with dynamic enhanced scan in the differential diagnosis of benign and malignant solitary pulmonary nodules.Methods A total of 40 patients with isolated pulmonary nodules admitted to People's Hospital of Suqian from July 2016 to July 2019 were selected.All patients were identified by 128-slice CT multi-sign analyzer combined with dynamic enhanced scan.The results of pathological examination or biopsy were used as the criteria.Results The sensitivity,specificity and accuracy of 128-slice CT combined with dynamic contrast-enhanced scanning in the diagnosis of benign and malignant solitary pulmonary nodules were 95.24%,89.47%and 92.50%,respectively.The enhancement value of CT in patients with benign and malignant pulmonary nodules was higher than that on plain CT scan in the same type,but the change range of CT value of malignant pulmonary nodules was larger(P<0.05).The PH value and SPH/PPH of malignant nodules were significantly higher than that of benign nodules(P<0.05).Conclusion The 128-slice CT multi sign analysis combined with dynamic contrast-enhanced scanning has a high resolution in the differential diagnosis of benign and malignant solitary pulmonary nodules,and the examination method is simple and quick,so it can be widely used in clinic.
