空心钉与关节置换术治疗老年股骨颈骨折的效果对比

Comparison of Hollow Nail and Joint Replacement in Treatment of Femoral Neck Fracture in the Elderly

目的观察对比空心钉内固定与关节置换术治疗老年股骨颈骨折患者的效果。方法回顾性分析2014年6月至2016年12月北京红十字会急诊抢救中心收治的60例老年股骨颈骨折患者,分为空心钉内固定组和关节置换术组,各30例。比较两组各手术指标、骨折不愈合情况。结果空心钉内固定组患者的手术时间长于关节置换术组(P<0.05),术中出血量少于关节置换术组(P<0.05),术中透视次数多于关节置换术组(P<0.05),术后下地时间长于关节置换术组(P<0.05),住院时间与关节置换术组相当(P>0.05),骨折不愈合率16.7%(5/30)高于关节置换术组0(P<0.05)。结论空心钉内固定与关节置换术治疗老年股骨颈骨折患者的效果均良好。

Objective To compare the effect of cannulated screw internal fixation and joint replacement in the treatment of elderly patients with femoral neck fracture.Methods From June 2014 to December 2016,60 elderly patients with femoral neck fracture were randomly divided into cannulated screw internal fixation group and joint replacement group,30 cases in each group.The operative indexes and nonunion of fracture were compared between the two groups.Results The operation time of cannulated screw internal fixation group was longer than that of joint replacement group(P<0.05),the amount of bleeding during operation was less than that of joint replacement group(P<0.05),the times of fluoroscopy during operation were more than that of joint replacement group(P<0.05),the time of landing after operation was longer than that of joint replacement group(P<0.05),hospitalization time and joint replacement group(P<0.05).The nonunion rate of fracture was 16.7%(5/30),higher than that of the joint replacement group(P<0.05).Conclusion Both cannulated screw internal fixation and arthroplasty are effective in the treatment of elderly patients with femoral neck fracture.
