血清微小RNA-96在老年慢性心力衰竭患者中的表达及意义 被引量:4

摘要 目的探究微小RNA-96(miR-96)在老年慢性心力衰竭(CHF)患者中的表达及其在老年CHF诊断中的意义。方法选取我院收治的CHF患者165例为CHF组,另选取健康志愿者50例为对照组,收集所有受试者一般临床资料,检查血清尿素、肌酐、C反应蛋白(CRP)和N末端B型钠尿肽前体(NT-proBNP)及miR-96表达水平。采用ROC曲线,计算血清miR-96诊断CHF的曲线下面积(AUC)。结果与对照组比较,CHF组血清尿素、肌酐、CRP和NT-proBNP及miR-96水平明显升高,LVEF明显降低(P<0.01)。CHF组血清miR-96水平与尿素、肌酐和NT-proBNP水平呈正相关(r=0.518,P=0.003;r=0.704,P=0.000;r=0.401,P=0.030),与LVEF呈负相关(r=-0.452,P=0.032),与CRP无相关性(P>0.05)。ROC曲线分析显示,血清miR-96水平诊断CHF的AUC为0.807(95%CI:0.752~0.861,P<0.05),预测CHF的特异性为83.2%,敏感性为82.4%。结论血清miR-96在CHF患者中高表达,且CHF患者的部分临床病理学参数也随之发生显著变化,血清miR-96有可能作为老年CHF患者诊断特异性分子标志物。 Objective To study the expression of circulating miR-96 in elderly chronic heart failure(CHF)patients and its significance in diagnosis of CHF.Methods One hundred and sixty-five CHF patients admitted to our hospital served as a CHF group and 50 healthy volunteers served as a control group.Their general clinical data were recorded,their serum levels of urea,creatinine,CRP,NT-proBNP and the expression of circulating miR-96 were measured.The relationship between circulating miR-96 and CHF was analyzed by Pearson correlation analysis and ROC curve analysis respectively.The area under the ROC curve for circulating miR-96 in diagnosis of CHF was calculated.Results The serum levels of urea,creatinine,CRP and NT-proBNP were significantly higher while the LVEF was significantly lower in CHF group than in control group(P<0.01).Pearson correlation analysis showed that serum levels of circulating miR-96 were positively correlated with those of urea,creatinine and NT-proBNP(r=0.518,P=0.003;r=0.704,P=0.000;r=0.401,P=0.030),negatively related with LVEF(r=-0.452,P=0.032)and not related with serum CRP level in CHF group(P>0.05).ROC curve analysis dispayed that the area under the ROC curve for circulating miR-96 in diagnosis of CHF was 0.807(95%CI:0.752-0.861,P<0.05).The specificity and sensitivity of circulating miR-96 in predicting CHF was83.2%and 82.4%respectively.Conclusion Circulating miR-96 is highly expressed in elderly CHF patients and some clinicopathological parameters of CHF patients will change significantly with circulating miR-96.Serum circulating MiR-96 level can thus be used as a specific molecular marker in diagnosis of CHF.
