晚断脐干预对产妇产后2h阴道出血的影响及改善措施分析

Analysis of the effect of delayed umbilical cord ligation on postpartum 2h vaginal bleeding and improvement measures

[目的]探讨并分析晚断脐干预对产妇产后2h阴道出血的影响及改善措施。[方法]选择某院2018年1月-2018年12月收治的138例足月阴道自然分娩的产妇作为研究对象,利用随机数字表法将其分为对照组和观察组各69例。对照组产妇在新生儿分娩后进行常规断脐,观察组产妇在新生儿分娩后进行晚断脐干预,在断脐后将脐部暴露直至干燥自然脱落,比较两组产妇产后2h阴道出血情况以及两组新生儿脐部并发症的发生率。[结果]两组产妇第三产程时间比较差异无统计学意义,但观察组产妇产后2h的出血量明显低于对照组(P<0.05);观察组新生儿脐带脱落时间明显早于对照组(P<0.05)。观察组新生儿脐部并发症总发生率为4.35%,明显低于对照组的24.64%(P<0.05)。[结论]晚断脐再加脐带自然干燥干预对产妇和新生儿的效果更好,可明显减少产妇产后2h阴道出血量,缩短新生儿脐带脱落时间,降低新生儿脐部并发症的发生率,安全性高,实用性强。

Objective:To explore and analyze the effect of delayed umbilical cord ligation on postpartum 2 h vaginal bleeding and its improvement measures.Methods:A total of 138 cases of parturients after full-term vaginal natural delivery admitting to ahospital from January 2018 to December 2018 were selected as research objects,and they were divided into control group and observation group by using the random number table method,69 cases in each group.In control group,the puerperas received routine umbilical cord ligation after delivery.In observation group,the puerperas received the delayed cord clamping intervention,and the umbilical cord was exposed until dry and natural fall off after the umbilical cord clamping.The postpartum 2 h vaginal bleeding amount of puerperas and the incidence of umbilical complications of newborns were compared between the two groups.Results:There was no statistically significant difference in the time of the third stage of labor between the two groups,but the amount of postpartum2 h bleeding in the observation group was significantly lower than that in the control group(P<0.05).The neonatal umbilical cord shedding time in the observation group was significantly earlier than that in the control group(P<0.05).The total incidence of umbilical complications of newborns was 4.35%in the observation group,which was significantly lower than 24.64%in the control group(P<0.05).Conclusions:Delayed umbilical cord ligation and umbilical cord natural drying intervention has abetter effect on puerperas and newborns.It can significantly reduce the amount of postpartum2 h vaginal bleeding,shorten the time of umbilical cord shedding and reduce the incidence of neonatal umbilical complications.It had high safety and practicability.
