微氧联合负压封闭辅助伤口闭合技术在骨科慢性难愈性创面中的应用效果 被引量:6

摘要 目的研究微氧联合负压封闭辅助伤口闭合技术治疗骨科慢性难愈性创面的临床效果。方法选取2017年1月至2018年7月中国人民解放军第九八八医院收治的30例骨科慢性难愈性创面患者作为研究对象,并按照随机数表法将其随机分为观察组(15例)与对照组(15例),其中观察组患者采用微氧联合负压封闭辅助伤口闭合技术治疗局部创面,对照组患者单纯采用负压封闭辅助伤口闭合技术治疗局部创面,对比两组患者创面渗液pH值、创面温度、肉芽组织覆盖率、临床疗效以及创面愈合时间。结果治疗10 d后,观察组患者创面渗液pH值明显低于对照组(t=3.587,P=0.001),而创面温度及肉芽组织覆盖率明显高于对照组(t=10.230、8.110,P均=0.000)。治疗2周后,观察组患者治疗总有效率为93.3%,明显高于对照组患者的治疗总有效率53.3%(χ^2=6.136,P=0.013)。最终两组患者创面均完全愈合,且观察组患者创面愈合时间明显短于对照组(t=2.119,P=0.043)。结论微氧联合负压封闭辅助伤口闭合技术治疗骨科慢性难愈性创面,可明显改善创面pH值、温度等微环境,有效促进肉芽组织生长,加快创面愈合,疗效显著,值得临床推广应用。 Objective To study the clinical effect of micro-oxygen combined with vacuum assisted closure in the treatment of chronic hard-to-healing orthopaedic wounds.Methods Thirty patients with chronic orthopaedic wounds,admitted to The People’s Liberation Army 988 th Hospital from January 2017 to July 2018,were selected as study subjects and then divided,according to the random number table,into an observation group(15 cases)and a control group(15 cases).Patients in the observation group were treated with micro-oxygen combined with vacuum assisted closure,while patients in the control group were treated with vacuum assisted closure alone.The pH value of wound exudate,wound temperature,granulation tissue coverage rate,clinical efficacy and wound healing time were compared between the two groups.Results After 10 days of treatment,the pH value of wound exudate in the observation group was significantly lower than that in the control group(t=3.587,P=0.001),while the wound temperature and granulation tissue coverage rate were significantly higher than those in the control group(t=10.230,8.110,all P=0.000).After 2 weeks of treatment,the total effective rate of the observation group was 93.3%,which was significantly higher than that of the control group(53.3%)(χ^2=6.136,P=0.013).Finally,the wounds of both groups healed completely and the wound healing time of the observation group was significantly shorter than that of the control group(t=2.119,P=0.043).Conclusion Micro-oxygen combined with vacuum assisted closure in the treatment of chronic hard-to-healing wounds can significantly improve the wound microenvironment,such as pH value and temperature,promote granulation tissue growth,accelerate wound healing.It deserves to be promoted in clinical application due to its good curative effect.
