针灸联合金匮肾气丸对脊髓损伤后神经源性膀胱尿潴留患者影响的临床研究

Effect of acupuncture combined with Jingui Shenqi Pills on patients with neurogenic bladder and urine retention after spinal cord injury

目的探讨研究针灸联合金匮肾气丸对脊髓损伤后神经源性膀胱尿潴留的治疗效果。方法选取2016年1月—2019年10月接受治疗的110例脊髓损伤后神经源性膀胱尿潴留患者为研究对象,随机分为对照组和治疗组,各55例,两组患者均进行间歇导尿方法治疗,对照组患者采用针灸方法治疗,治疗组在对照组基础上加服金匮肾气丸,记录治疗前后患者膀胱压力容量、膀胱最大容量和膀胱残余尿量,采用生活质量指数评价患者生活质量,并记录治疗疗效。结果治疗前,两组患者膀胱压力容量、膀胱最大容量和膀胱残余尿量差异无统计学意义(P>0.05),治疗后,两组患者膀胱残余尿量、膀胱最大容量均降低,膀胱压力均增大,差异有统计学意义(P<0.05),且治疗组患者膀胱最大容量、膀胱残余尿量均小于对照组,膀胱压力大于对照组,差异有统计学意义(P<0.05);治疗前两组患者生活质量指数得分差异无统计学意义(P>0.05),治疗后两组患者生活质量指数得分均降低,差异有统计学意义(P<0.05),且治疗组患者治疗后生活质量指数得分低于对照组,差异有统计学意义(P<0.05);治疗组患者总有效率92.73%,高于对照组的67.27%,差异有统计学意义(P<0.05)。结论针灸联合金匮肾气丸能够降低脊髓损伤后神经源性膀胱尿潴留患者残余尿量、膀胱最大容量,增大膀胱压力,提高治疗效果,提高患者生活质量。

Objective To investigate the therapeutic effect of acupuncture combined with Jingui Shenqi Pills on neurogenic bladder urinary retention after spinal cord injury.Methods A total of 110 patients with neurogenic bladder urinary retention after spinal cord injury treated from January,2016 to October,2019 were included and randomly divided into a control group and a study group,with 55 patients in each group.Intermittent catheterization was applied to patients in both groups.The control group was treated with acupuncture and the study group was given Jingui Shenqi Pills combined with acupuncture.The bladder pressure capacity,bladder maximum volume and bladder residual urine volume in both groups were recorded before and after treatment.The quality of life index was assessed and the clinical efficacy was evaluated.Results Before treatment,there was no significant difference in bladder pressure capacity,maximum bladder volume and bladder residual urine volume between the two groups(P>0.05).After treatment,bladder residual urine volume and maximum bladder capacity decreased in both groups,and bladder pressure increased,and the difference was statistically significant(P<0.05);the maximum bladder volume and bladder residual urine volume in the study group were lower than in the control group,and the bladder pressure in the study group was larger than in the control group,with statistical significance(P<0.05).Before treatment,there was no significant difference in the quality of life index between the two groups(P>0.05).After treatment,the quality of life index score in both groups decreased,with statistical significance(P<0.05),and the quality of life index score in the study group was lower than in the control group,with statistical significance(P<0.05).The total effective rate of the study group(92.73%)was higher than that of the control group(67.27%),with statistical significance(P<0.05).Conclusion For patients with neurogenic bladder and urine retention after spinal cord injury,acupuncture combined with Jingui Shenqi Pills can reduce residual urine volume,maximum bladder capacity,increase bladder pressure,increase the therapeutic effect and improve the quality of life.
