软式支气管镜在小儿难治性肺炎支原体肺炎治疗中的应用

Application of Soft Bronchoscope in the Treatment of Refractory Mycoplasma Pneumoniae Pneumonia in Children

目的探寻软式支气管镜在小儿难治性肺炎支原体肺炎(RMPP)治疗中的应用价值。方法以2015年1月至2018年12月本院儿童呼吸内科住院的97例确诊RMPP病例为研究对象,收集临床表现、实验室检查、影像学特征、治疗疗效等临床资料,比较支气管镜组与非支气管镜组治疗后热峰、发热持续时间、住院时间、炎性指标、肺部并发症恢复情况,探寻软式支气管镜在RMPP治疗中的作用。结果支气管镜组热峰下降迅速,发热持续时间短;住院时间明显缩短;炎性指标下降快;肺部并发症恢复良好。非支气管镜组的临床情况较支气管镜组恢复缓慢;组间差异有统计学意义(P<0.05)。结论在RMPP患儿治疗中,选择适宜的适应征,软式支气管镜的运用具有重要的临床价值。

Objective To explore the value of soft bronchoscopy in the treatment of pediatric refractory mycoplasmal pneumoniae pneumonia(RMPP).Methods From January 2015 to December 2018,97 confirmed RMPP cases in our hospital's children's respiratory medicine hospital were taken as the research object,and clinical data such as clinical manifestations,laboratory tests,imaging features,and therapeutic effects were collected.The heat peak,duration of fever,length of hospital stay,inflammatory indexes,and recovery of pulmonary complications were compared between the bronchoscopy group and the non-bronchoscopic group,and the role of soft bronchoscopy in RMPP treatment was explored.Results In the bronchoscopy group,(1)the heat peak decreased rapidly and the duration of fever was short;(2)the length of hospital stay was significantly reduced;(3)the inflammatory index decreased rapidly;and(4)the pulmonary complications recovered well.The clinical situation of the non-bronchoscopic group was slower than that of the bronchoscopic group.The difference between the groups was statistically significant(P<0.05).Conclusion In the treatment of children with RMPP,the selection of appropriate indications and the use of soft bronchoscopes have important clinical value.
