针刀整体松解术治疗脑瘫膝外翻畸形的临床效果

Clinical effect of acupotomy in the treatment of genu valgus deformity of cerebral palsy

目的探讨针刀整体松解术治疗脑瘫膝外翻畸形的临床效果。方法选取2016年3月~2019年10月于湖北中医药大学黄家湖医院针刀科应用针刀松解膝关节周围肌肉、韧带、筋膜的粘连、瘢痕、挛缩治疗的12例脑瘫膝外翻患者。对患者治疗前后股胫角(FTA)外翻角、膝关节屈伸活动范围及有效率进行评估。结果治疗后FTA外翻角明显小于治疗前,差异有统计学意义(P<0.05)。治疗后膝关节屈伸活动范围明显大于治疗前,差异有统计学意义(P<0.05),总有效率为91%。结论针刀治疗膝外翻的原理是:松解膝关节周围软组织,改变FTA外翻角,矫正畸形;增大膝关节屈伸活动范围,明显改善患者膝关节的功能。疗效显著,临床应优先考虑针刀保守治疗。

Objective To evaluate clinical efficacy of acupotomy in the treatment of genu valgus deformity of cerebral palsy.Methods From March 2016 to October 2019,12 patients with genu valgus of cerebral palsy were selected from the Department of Acupotomology of Huangjiahu Hospital in Hubei University of Chinese Medicine to use acupotomology to release the adhesion,scar and contracture of the muscles,ligaments and fascia around the knee joint.Valgus angle of femoral tibial angle(FTA),range of flexion and extension of knee joint and effective rate before and after treatment were evaluated.Results FTA valgus angle of patients after treatment was significantly smaller than that before treatment,the difference was statistically significant(P<0.05).After treatment,the range of flexion and extension of knee joint was significantly larger than that before treatment,the difference was statistically significant(P<0.05),and total effective rate was 91%.Conclusion The principle of acupotomotor in the treatment of genu valgus is to loosen the soft tissue around the knee joint,change the FTA valgus angle,correct the deformity;to increase the range of flexion and extension of the knee joint,so as to significantly improve the function of the knee joint of the patient.The curative effect is significant,it should give priority to acupotomotor conservative treatment in clinic.
