北京同仁堂中医医院2013-2018年门诊中成药使用情况分析

Analysis on Use of Chinese Patent Medicine in Outpatient Department of Beijing Tongrentang Traditional Medicine Hospital from 2013 to 2018

目的分析北京同仁堂中医医院(以下简称“本院”)门诊中成药用药现状和趋势,为临床合理使用中成药提供参考。方法采用回顾性调查方法,调取本院信息系统2013-2018年门诊中成药相关数据,统计中成药品种、用量、销售金额,计算其年增长率、用药频度(DDDs)、限定日费用(DDC)。结果门诊中成药使用占整体门诊药物使用比例变化较小,稳定在40%;2013-2016年,门诊中成药处方量及销售金额整体呈逐年增长、增速放缓趋势,受2017年北京市医药分开综合改革影响,门诊中成药处方量下降4.93%、销售金额下降7%,2018年中成药处方量增长率2.43%、销售金额较2017年增长1%。门诊中成药销售金额排序稳定,与门诊中成药销售类别排序趋同。6年内门诊中成药DDDs呈整体上升趋势,排序前3位为金水宝胶囊、复方丹参滴丸、同仁牛黄清心丸。结论本院2013-2018年门诊中成药应用情况较为理想,基本涵盖了本院就医人群的中成药用药需求,但需进一步加强管理不合理用药问题。

Objective To analyze the current situation and trend of use of Chinese patent medicine in Beijing Tongrentang Traditional Medicine Hospital(hereinafter referred to as“our hospital”);To provide references for the rational use of Chinese patent medicine in clinical practice.Methods Retrospective survey method was adopted to collect relevant data of Chinese patent medicine in outpatient of our hospital from 2013 to 2018 from the hospital information system.The varieties,dosage and sales amount of Chinese patent medicine were statistically analyzed,and the annual growth rate,drug frequency(DDDs)and daily drug cost(DDC)of Chinese patent medicine were calculated.Results The proportion of the use of Chinese patent medicine in outpatient in the whole outpatient service changed little and remained at 40%.From 2013 to 2016,the prescription and sales amount of Chinese patent medicine in outpatient increased year by year,and the growth rate slowed down.Affected by the comprehensive and separated reform of Beijing pharmaceuticals in 2017,the use of Chinese patent medicines in outpatient declined partially:the prescription volume decreased by 4.93%,and the sales amount decreased by 7%.In 2018,the prescription growth rate was 2.43%;the sales amount increased by 1%compared with 2017.The ranking of sales amount of outpatient Chinese patent medicine tended to be stable,which was similar to that of sale categories of outpatient Chinese patent medicine.DDDs of Chinese patent medicine in outpatient showed an overall rising trend within 6 years,and the top three in the ranking frequency were:Jinshuibao Capsules,compound Danshen Dropping Pills,and Tongren Niuhuang Qingxin Pills.Conclusion The application of Chinese patent medicine in outpatient of our hospital in 2013–2018 is quite satisfactory.The outpatient Chinese patent medicines basically cover the demand for Chinese patent medicine in the outpatient of our hospital,and it is necessary to further strengthen the management of irrational medication.
