吴良村治疗肺癌气虚证的疗效评价及用药规律

Analysis of Curative Efficacy and Prescribing Rules of WU Liangcun in Treating Qi Deficiency Syndrome in Lung Carcinoma

[目的]探究吴良村教授诊治肺癌气虚证的疗效及用药规律。[方法]从吴良村教授的门诊病案中选择80例肺癌气虚证患者,四诊合参、辨证施治,以2周为1个周期,治疗1个月后评价疗效。同时对160例医案中的药物频数、性味、分类进行描述性分析,并对用药频数最多的前33味中药进行系统聚类分析,总结其用药规律。[结果]患者临床疗效显著,总有效率达88.75%。用药频数最多的为甘草。药性使用频数最多的为平性药,药味使用频数最多的为甘味药,用药种类最多的为补虚药。聚类分析将药物分为19类,包括沙参麦冬汤、补中益气汤及经验药对等。[结论]吴良村教授治疗肺癌气虚证的过程中善用沙参麦冬汤加减,宗"益气养阴,解毒散结"之法,以"补、清、散、消"为用药原则,临床取得显著疗效。

[Objective]To explore the curative efficacy and the prescribing rules of Professor WU Liangcun’s prescriptions in treating Qi deficiency syndrome in lung carcinoma.[Methods]Eighty lung carcinoma patients of Qi deficiency syndrome were selected from Professor WU Liangcun’s outpatients,and the diagnosis and treatment were based on synthesis of the four diagnostic methods and overall analysis of the illness and the patient’s condition.Every two weeks made a period of treatment,and the treatment effect was evaluated after two periods.Furthermore,description analysis was developed about the properties,tastes,type of herbal medicine and cluster analysis was developed about the most frequently used 33 herbal medicines,which were extracted from 160 prescriptions,in order to summarize the prescription rules of Professor WU Liangcun in treating Qi deficiency syndrome in lung carcinoma.[Results]Patients achieved significant clinical efficacy,with a total effective rate of 88.75%.The most frequently used herbal medicine was Glycyrrhizae.The gentle property,the sweet taste and the tonic type of herbal medicines were used most frequently.Cluster analysis divided the herbal medicines into 19 categories,including Shashen Maidong Decoction,Buzhong Yiqi Decoction and experienced herbal medicines pairs.[Conclusion]WU Liangcun is good at using Shanshen Maidong Decoction to treat Qi deficiency syndrome in lung carcinoma.Taking"tonifying,clearing,dispersing and eliminating"as the principle of medication,the methods of supplementing Qi and nourishing Yin,detoxificating and eliminating mass were adopted,and good clinical efficacy was achieved.
