夏永良运用半夏厚朴汤验案五则

Five Medical Cases of Professor XIA Yongliang’s Clinical Experience in Using Banxia Houpu Decoction

[目的]总结夏永良老师临床应用半夏厚朴汤的经验。[方法]通过跟随夏永良老师临诊并查阅夏永良老师医案,选取夏师治疗咳嗽、慢性咽炎、胃炎、睡眠障碍、围绝经期综合征之验案5则,并结合历代名家的相关论述,从病因病机、辨证论治、用药特点等方面,分析总结夏师对于半夏厚朴汤的学术观点及临床经验。[结果]夏永良老师认为临床运用半夏厚朴汤需抓住气机郁滞、痰气互结这一病机。夏师临床常将半夏厚朴汤与小陷胸加枳实、四逆散、小柴胡汤、升降散等合用,夏师认为只要辨证准确,此方不仅可用来治疗梅核气,也可用于治疗其他疾病且能取得良好疗效。[结论]夏永良老师从气机郁滞、痰气互结出发,灵活运用半夏厚朴汤治疗各类疾病的经验,临床疗效肯定,值得学习与推广应用。

[Objective]To summarize the experience of Professor XIA Yongliang’s clinical application of Banxia Houpu decoction.[Methods]From following Professor XIA Yongliang clinical studies,studying Professor XIA Yongliang’s medical cases and selecting medical records on cough,chronic pharyngitis,gastritis,sleep disorder and perimenopause syndrome,we can study the aspects of etiology and pathogenesis,syndrome differentiation treatment,prescriptions,drug characters,then summarize his thoughts and experience on using Banxia Houpu decoction.[Results]Professor XIA Yongliang believes that in the clinical application of Banxia Houpu decoction,it is necessary to grasp the pathogenesis of Qi stagnation and combination of Tan and Qi.He often combines Banxia Houpu decoction with Xiaoxianxiong plus Zhishi decoction,Sini powder,Xiaochaihu decoction and Shengjiang powder etc,and he believes that Banxia Houpu decoction can not only be used to treat globus hysteriocus but also other diseases as long as we carefully analyse the diseases.[Conclusions]Professor XIA Yongliang’s experience on treating different kinds of gynecological diseases by Banxia Houpu decoction has positive effect and is worth further study.
