纤维支气管镜下液基细胞学检测技术在肺癌诊断及分型中的应用价值 被引量:8

Application value of fiber bronchoscopic liquid-based cytology test technology in the diagnosis and classification of lung cancer

摘要 目的探讨纤维支气管镜下液基细胞学检测(LCT)技术在肺癌诊断及分型中的应用价值。方法收集193例可疑肺癌患者的临床资料,所有患者均行纤维支气管镜下LCT。以活检或术后组织病理学结果为金标准,分析LCT技术对肺癌的诊断价值。结果经活检或术后病理诊断为肺癌165例,良性病变28例。LCT诊断肺癌的灵敏度为93.3%(154/165),特异度为89.3%(25/28),诊断符合率为92.7%(179/193)。在肺癌分型中,其对鳞癌、腺癌、小细胞肺癌、未分型癌的诊断灵敏度分别为60.3%(41/68)、77.8%(42/54)、69.6%(16/23)、70.0%(14/20),特异度分别为95.9%(93/97)、93.7%(104/111)、98.6%(140/142)、80.7%(117/145),诊断符合率分别为81.2%(134/165)、88.5%(146/165)、94.5%(156/165)、79.4%(131/165)。结论纤维支气管镜下LCT对肺癌的诊断及分型的特异度及准确性均较高,且检查方法简便、可靠,是行之有效的肺癌诊断、分型方法。 Objective To explore the application value of fibeRbronchoscopic liquid-based cytology test(LCT)technology in the diagnosis and classification of lung cancer.Methods The clinical data of 193 patients with suspected lung canceRwere collected,and all patients underwent fibeRbronchoscopic LCT.With the biopsy or postoperative pathologic results as gold standard,the diagnostic value of LCT technology foRlung canceRwas analyzed.Results A total of 165 cases of lung canceRand 28 cases of benign lesions were identified by biopsy oRpostoperative pathological examination.The sensitivity of LCT in the diagnose of lung canceRwas 93.3%(154/165),the specificity was 89.3%(25/28),and the diagnostic coincidence rate was 92.7%(179/193).In the classification of lung cancer,the diagnostic sensitivities of LCT foRsquamous cell carcinoma,adenocarcinoma,small cell lung cancer,and untyped carcinoma were 60.3%(41/68),77.8%(42/54),69.6%(16/23),and 70.0%(14/20),respectively,the specificities were 95.9%(93/97),93.7%(104/111),98.6%(140/142),and 80.7%(117/145),respectively,the diagnostic coincidence rates were 81.2%(134/165),88.5%(146/165),94.5%(156/165),and 79.4%(131/165),respectively.Conclusion FibeRbronchoscopic LCT has higheRspecificity and accuracy in the diagnosis and classification of lung cancer,in addition,the inspection method is simple and reliable,and it is an effective approach foRlung canceRdiagnosis and classification.
