大剂量奥美拉唑治疗老年胃溃疡合并出血的临床效果观察

Clinical Observation of Large Dose Omeprazole in the Treatment of Elderly Gastric Ulcer complicated with Hemorrhage

目的观察大剂量奥美拉唑治疗老年胃溃疡合并出血的临床效果。方法选择我院在2018年1月至2019年2月期间收治的60例老年胃溃疡合并出血患者,依据治疗方案分为对照组(n=30例)与观察组(n=30例),对照组患者应用常规剂量奥美拉唑治疗方案,观察组患者应用大剂量奥美拉唑治疗方案,对比分析两组患者的治疗效果、治疗后再出血情况以及治疗期间不良反应发生情况。结果观察组患者的治疗总有效率96.67%(29/30)明显高于对照组患者的治疗总有效率73.33%(22/30),P<0.05;观察组患者治疗后3 d内再出血率3.33%(1/30)、1个月内再出血率0.00%(0/30)、3个月内再出血发生率0.00%(0/30)均显著低于对照组患者13.33%(4/30)、10.00%(3/30)、6.67%(2/30),P<0.05;观察组患者中不良反应发生率为6.67%(2/30),对照组患者中不良反应发生率为10.00%(3/30),两组患者不良反应发生率对比差异不明显,P>0.05。结论针对老年胃溃疡合并出血应用大剂量奥美拉唑治疗效果尤为显著,能够有效减少患者再次出血的风险,缓解其临床症状,且安全可靠性高,有助于促进患者病情快速恢复,进而改善生活质量,在临床上具有应用价值。

Objective To observe the clinical effect of high-dose omeprazole in the treatment of elderly gastric ulcer complicated with hemorrhage.Methods Select our hospital from January 2018 to February 2019 in admitted during the period of 110 cases of elderly patients with gastric ulcer with bleeding,on the basis of treatment were divided into control group(n=30 cases)and observation group(n=30 cases)and control group patients with regular doses of omeprazole treatment,observation group of patients using high-dose omeprazole treatment,compared the therapeutic effect of two groups of patients,treatment after bleeding and adverse reactions occurred during treatment.Results The total effective rate of treatment in the observation group was 96.36%(53/30),significantly higher than that in the control group78.18%(43/30),P<0.05.In the observation group,the re-bleeding rate was 1.82%(1/30)within 3 days after treatment,0.00%(0/30)within 1 month,0.00%(0/30)within 3 months,and the re-bleeding rate was significantly lower than that of the control group14.30%(8/30),9.09%(5/30),and 10.91%(6/30),P<0.05.The incidence of adverse reactions was 5.45%(3/30)in the observation group and 3.64%(2/30)in the control group.There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The application of large dose omeprazole in the treatment of elderly gastric ulcer complicated with hemorrhage is particularly effective,which can effectively reduce the risk of patients’re-bleeding,alleviate their clinical symptoms,and has high safety and reliability,which is helpful to promote the rapid recovery of patients’conditions and improve the quality of life,and has clinical application value.
