摘要
目的:利用3.0T MR动态增强扫描探讨乳腺癌周围血管特征与乳腺癌预后因素的相关性。方法:选取2018年1—12月在汕头大学医学院附属肿瘤医院行MR动态增强扫描并经手术或穿刺活检病理确诊的102例乳腺癌患者。采用乳腺MR动态增强3D-最大密度投影血管减影成像技术评价乳腺癌相关血管特征,分析肿瘤周围血管特征与肿瘤大小、组织病理学分级、人表皮生长因子受体2(HER-2)、孕激素受体(PR)和雌激素受体(ER)等预后因素的相关性。结果:102例乳腺癌患者中,45例肿瘤周围血管征阳性,61例患侧乳房与对侧正常乳房相比,血管增多明显。肿瘤周围血管征与肿瘤大小、ER、PR、HER-2差异均有统计学意义(均P<0.05),与组织病理分级差异没有统计学意义(P>0.05);患侧乳腺血管增多与肿瘤大小、ER、PR差异有统计学意义(均P<0.05)。结论:患侧乳腺血管增多和肿瘤周围血管征阳性与乳腺癌预后因素存在相关性,可为临床评价乳腺癌患者预后提供参考。
Objective:Dynamic contrast-enhanced 3.0T MR was used to study the relationship between the characteristics of peripheral blood vessels and the prognosis of breast cancer.Methods:From January to December 2018,102 patients with breast cancer were selected from the Cancer Hospital of Shantou University Medical College for MR dynamic enhancement scanning and confirmed by operation or biopsy.The 3D-MIP technique was used to evaluate the vascular characteristics of breast cancer.The correlation between the vascular characteristics of breast cancer and the prognostic factors such as tumor size,histopathological grade,human epidermal growth factor receptor-2(HER-2),progesterone receptor(PR)and estrogen receptor(ER)was analyzed.Results:Among 102 cases of breast cancer,45 cases were positive for peritumoral vascular sign,and 61 cases had significantly more vessels in the affected breast compared with the contralateral normal breast.There were significant differences between the tumor size,ER,PR,HER-2 and the peripheral vascular sign(P<0.05).There were statistically significant differences in tumor size,ER and PR between the increased vessels in the affected breast(P<0.05).Conclusion:There is a correlation between the positive sign of blood vessels around the tumor and the prognosis factors of breast cancer,which can provide a reference for clinical evaluation of the prognosis of breast cancer patients.
