益生菌联合铋剂四联疗法根除幽门螺杆菌的临床研究

Clinical research on probiotics combined with bismuth quadruple therapy in eradication of helicobacter pylori

目的探讨益生菌联合铋剂四联疗法根除幽门螺杆菌的临床疗效。方法选取本院2016年10月~2018年10月幽门螺杆菌感染的慢性胃炎患者176例,采用随机数字表法分为两组,对照组88例采用铋剂四联疗法,观察组88例采用益生菌联合铋剂四联疗法,于治疗前后行生化指标检测、临床症状评定,比较两组治疗效果、幽门螺杆菌根除效果、不良反应情况。结果两组治疗后白细胞介素-2、前列腺素E2、热休克蛋白较治疗前显著增加(P<0.05)。两组治疗后白细胞介素-4、白细胞介素-10、临床症状评分(腹痛积分、嗳气积分、反酸积分、腹胀积分、少食积分、总评分)较治疗前显著降低(P<0.05)。观察组治疗后白细胞介素-2、前列腺素E2、热休克蛋白高于对照组(P<0.05)。观察组治疗后白细胞介素-4、白细胞介素-10、临床症状评分(腹痛积分、腹胀积分、总评分)低于对照组(P<0.05)。观察组总有效率、幽门螺杆菌根除率高于对照组(P<0.05)。两组不良反应(恶心呕吐、头晕头痛、胃肠道反应、过敏反应)比较,差异无统计学意义(P>0.05)。结论益生菌联合铋剂四联疗法根除幽门螺杆菌的效果显著,值得临床推广应用。

Objective To explore clinical curative effects of probiotics combined with bismuth quadruple therapy in eradication of helicobacter pylori.Methods 176 patients with chronic gastritis due to helicobacter pylori infection were selected in hospital from October 2016 to October 2018.They were divided into two groups according to the random number table.88 patients in the control group were given bismuth quadruple therapy while 88 patients in the observation group were given probiotics combined with bismuth quadruple therapy.Biochemical indexes and clinical symptoms were assessed before and after treatment.Curative effects,eradication effect of helicobacter pylori and adverse reactions were compared between two groups.Results Interleukin-2,prostaglandin E2 and heat shock protein increased significantly in two groups after treatment(P<0.05).Interleukin-4,Interleukin-10,clinical symptom scores(abdominal pain score,belching score,acid reflux score,abdominal distension score,anorexia score,total score)decreased significantly in two groups after treatment(P<0.05).After treatment,interleukin-2,prostaglandin E2,heat shock protein after treatment in the observation group were higher than those in the control group(P<0.05).After treatment,interleukin-4,interleukin-10,clinical symptom scores(abdominal pain score,abdominal distension score,total score)in observation group were lower than those in the control group(P<0.05).Total effective rate and eradication rate of helicobacter pylori in observation group were higher than those of the control group(P<0.05).There were no statistically significant differences in adverse reactions(nausea and vomiting,dizziness and headache,gastrointestinal reactions,allergic reactions)between two groups(P>0.05).Conclusion Probiotics combined with bismuth quadruple therapy have significant effect in eradication of helicobacter pylori,which is worthy of clinical promotion and application.
