布地奈德雾化联合孟鲁司特钠治疗小儿咳嗽变异性哮喘对全身炎症反应影响

Effects of budesonide atomization combined with Montelukast sodium in the treatment of children with cough variant asthma on systemic inflammatory response

目的探讨布地奈德雾化联合孟鲁司特钠治疗小儿咳嗽变异性哮喘疗效及全身炎症反应影响。方法选取2017年2月~2018年11月我院诊治咳嗽变异性哮喘患儿76例进行研究,随机分为常规组(常规抗感染、抗炎及布地奈德雾化治疗,38例)和干预组(常规治疗基础上联合孟鲁司特钠治疗,38例),比较两组患儿疗效及治疗前后白细胞介素-8(IL-8)、白细胞介素-4(IL-4)、肿瘤坏死因子(TNF-α)水平。结果干预组患者治疗总有效率显著高于常规组(97.37%vs.81.58%,P<0.05)。治疗前,两组患者IL-8、IL-4及TNF-α水平比较,差异无统计学意义(P>0.05);治疗4周,相对治疗前,常规组和干预组患者IL-8、IL-4及TNF-α水平显著降低(P<0.05);但干预组患者下降程度更显著(P<0.05)。结论布地奈德雾化联合孟鲁司特钠治疗小儿咳嗽变异性哮喘能减轻患儿全身炎症反应,提高临床疗效。

Objective To investigate the efficacy of budesonide atomization combined with Montelukast sodium in the treatment of children with cough variant asthma and the effects on systemic inflammatory response.Methods 76 children with cough variant asthma diagnosed and treated in our hospital from February 2017 to November 2018 were selected for the study,and randomly divided into the conventional group(conventional anti-infectious,antiinflammatory and budesonide atomization therapies,38 cases)and the intervention group(combined with Montelukast sodium therapy based on the conventional therapy,38 cases)by the random grouping table method.The efficacy and levels of interleukin-8(IL-8),interleukin-4(IL-4)and tumor necrosis factor(TNF-α)before and after treatment were compared between the two groups.Results The overall response rate of the patients in the intervention group was significantly higher than the conventional group[97.37%vs.81.58%,P<0.05].Before treatment,there were no significant differences in the levels of IL-8,IL-4 and TNF-α between the two groups of patients(P>0.05);After 4 weeks of treatment,the levels of IL-8,IL-4 and TNF-α in the conventional group and the intervention group of patients were significantly lower than those before treatment(P<0.05);However,the degree of decline was more significant in the intervention group of patients(P<0.05).Conclusion Budesonide atomization combined with Montelukast sodium in the treatment of children with cough variant asthma can alleviate systemic inflammatory response in children,and improve clinical efficacy.
