短节段椎弓根钉固定术对脊椎骨折患者脊髓功能的影响 被引量:3

Influence of Short-Segment Pedicle Screw Fixation on Spinal Cord Function in Patients with Spinal Fracture

摘要 目的探讨短节段椎弓根钉固定术治疗脊椎骨折的效果及对脊髓功能影响。方法选取2016年3月至2019年2月我院收治的50例脊椎骨折患者,根据手术方式分为A组(短节段椎弓根钉固定术,25例)和B组(跨伤椎椎弓根螺钉固定术,25例),比较两组患者的围手术期指标、Cobb角、伤椎前缘高度比及脊髓神经功能。结果A组的手术时间长于B组,术中出血量高于B组(P均<0.05)。两组患者的住院时间比较无统计学差异(P>0.05)。术前,两组的Cobb角、伤椎前缘高度比比较无统计学差异(P>0.05);术后6个月,两组的Cobb角低于术前,伤椎前缘高度比高于术前(P均<0.05);A组的Cobb角低于B组(P<0.05)。A组的脊髓功能严重程度低于B组(P<0.05)。结论短节段椎弓根钉固定术治疗脊椎骨折可提高患者的脊髓功能。 Objective To explore the effect of short-segment pedicle screw fixation on spinal fracture and the influence on spinal cord function.Methods 50 cases of patients with spinal fracture admitted to our hospital from March 2016 to February 2019 were selected and divided into group A(short-segment pedicle screw fixation,25 cases)and group B(transpedicular screw fixation,25 cases)according to operation methods.The perioperative indicators,Cobb angle,injured anterior border height ratio and spinal cord nerve function were compared between the two groups.Results The operation time of group A was longer than that of group B,and the intraoperative blood loss was higher than that of group B(both P<0.05).No statistical difference was found in the hospitalization time between the two groups(P>0.05).Before operation,no statistical difference was found in Cobb angle and injured anterior border height ratio between the two groups(P>0.05).6 months after operation,the Cobb angle of the two groups were lower than those before operation,and the injured anterior border height ratio were higher than those before operation(all P<0.05);The Cobb angle of group A was lower than that of group B(P<0.05).The severity degree of spinal cord function in group A was lower than that in group B(P<0.05).Conclusions Short-segment pedicle screw fixation in the treatment of spinal fracture can improve the spinal cord function of patients.
