颅脑损伤后精神障碍发生情况及影响因素分析

Analysis on the Occurrence and Influencing Factors of Mental Disorders after Craniocerebral Injury

目的研究颅脑损伤后精神障碍发生情况和影响因素。方法选取2015年10月至2019年9月我院收治的外伤所致颅脑损伤患者4921例,分析颅脑损伤后精神障碍发生率,将并发精神障碍患者设为A组,并在未发生精神障碍颅脑损伤患者中随机选取100例作为B组,收集两组患者相关临床资料和可疑影响因素,分析颅脑损伤后精神障碍发生影响因素。结果4921例颅脑损伤患者中,并发精神障碍48例(0.98%)。单因素分析显示,两组患者损伤到入院时间、入院时GCS评分、额叶损伤、颞叶损伤、脑干损伤、损伤程度、颅内血肿、损伤后昏迷比较差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,损伤到入院时间、入院时GCS评分、额叶损伤、损伤程度、颅内血肿为影响颅脑损伤后精神障碍发生独立危险因素。结论颅脑损伤后精神障碍发生影响因素较多,临床中应根据相关影响因素积极预防和控制,以降低颅脑损伤后精神障碍发生率。

Objective To study the occurrence and influencing factors of mental disorders after craniocerebral injury.Methods 4921 cases of patients with craniocerebral injury due to trauma admitted to our hospital from October 2015 to September 2019 were selected.The incidence of mental disorders after craniocerebral injury was analyzed.Patients complicated with mental disorders were set as group A,and 100 patients without mental disorders were randomly selected as group B.The clinical data and suspicious influencing factors of the two groups were collected to analyze the influencing factors of mental disorders after craniocerebral injury.Results Among 4921 patients,48 cases were complicated with mental disorders,with the incidence of 0.98%.Univariate analysis showed statistical difference was found in the time from injury to admission,GCS score at admission,frontal lobe injury,temporal lobe injury,brain stem injury,injury degree,intracranial hematoma,and coma after injury between the two groups(P<0.05).Multivariate Logistic regression analysis showed the time from injury to admission,GCS score at admission,frontal lobe injury,injury degree and intracranial hematoma were independent risk factors for the occurrence of mental disorders after craniocerebral injury.Conclusions As for many influencing factors of mental disorders after craniocerebral injury,it is necessary to actively prevent and control according to related influencing factors in clinical practice,so as to reduce the incidence of mental disorders after craniocerebral injury.
